摘 要:目的观察骨唾液蛋白和基质金属蛋白酶-9在风湿性心脏病瓣膜的表达。方法将手术切除的二尖瓣瓣膜按病史分为风湿性心脏病(风心病)组(120例)和非风心病组(30例),采用SP法进行免疫组化染色观察骨唾液蛋白和基质金属蛋白酶-9的表达。结果风心病组的骨唾液蛋白(91.6%)和基质金属蛋白酶-9(90.8%)阳性率明显高于非风心病组骨唾液蛋白(23.3%)和基质金属蛋白酶-9(20.0%)(x2值分别为56.6354、59.4272,P均〈0.01)。结论风心病瓣膜的钙化与基质金属蛋白酶-9所引起的细胞外基质降解和重塑以及骨唾液蛋白所诱导的成骨样骨形成密切相关。Objective To investigate the expression and significance of bone sialoprotein and matrix metalloproteinase-9 in calcified mitral valves in patients with rheumatic heart disease. Methods A total of 150 mitral valves were divided into the rheumatic group( n = 120) and the non-rheumatic group (n = 30). Expressions of bone sialoprotein and matrix metalloproteinase-9 were determined by immunohistochemistry. Results Expressions of bone sialoprotein ( 91.6% , ~2 = 56. 6354 ) and matrix metallowoteinase-9 ( 90. 8% , x2 = 59.4272) in the rheumatic group increased significantly than in the non-rheumatic group (P 〈 0.01). Conclusion Both bone sialoprotein and matrix metalloproteinase-9 are highly expressed in the calcified rheumatic group. This suggests that caficify of rheumatic mitral valves is related with the degradation and remodeling of extra cellular matricx by matrix metalloproteinase-9, as well as osteoblastlike bone formation by bone sialoprotein.
