摘 要:目的应用二维斑点追踪显像技术(2D-STI)评价MVP患者的左心房(LA)功能。方法利用Philips QLab 3DQA软件,获得左心房最大容积(LAV_(max))、左心房整体射血分数(LATEF),根据左心房最大容积(LAV_(max)),将46例MVP患者分成三组。利用TMQA软件,获得LA壁各节段的左心室收缩期峰值应变率(SRs)、左心室舒张早期峰值应变率(SRe)、左心室舒张晚期的峰值应变率(SRa),并计算各组中其平均值(mSRs、mSRe、mSRa)。结果 mSRa:组Ⅱ高于组Ⅰ、组Ⅲ;mSRe:组Ⅱ低于组Ⅰ、组Ⅲ;SRs:组Ⅰ与组Ⅱ比较,差异无统计学意义,组Ⅰ、组Ⅱ高于组Ⅲ。LATEF:组Ⅰ高于组Ⅱ、组Ⅲ,组Ⅱ高于组Ⅲ。观察者之间的一致性Kappa=0.82。结论 2D-STI技术可以评价MVP患者LA功能的变化。Objective To evaluate left atrial function with two-dimensional speckle tracking imaging (2D-STI) in patients with MVP. Methods Left atrial maximum volume (LAVmax) and left atrial total ejection fraction (LATEF) were acquired by real-time three dimensional echocardiography (RT 3DE). According to LAVmax, 46 patients were classified into three groups. The peak strain rate of systole, early and late diastole (SRs, SRe, SRa) of left atrial wall were measured with ZD-STI, and the mean peak strain rates (mSRs, mSRe and mSRa) were calculated. Results mSRa was higher in group Ⅱ than in group Ⅰand group Ⅲ. mSRe in group Ⅱ was lower than group I and group Ⅲ. SRs, there was no significant difference between groupⅠ and group Ⅱ , group Ⅲ was lower than group I and group Ⅱ. LATEF, group Ⅰ was higher than group Ⅱ and group Ⅲ , group Ⅱ was higher than group Ⅲ. The interobserver agreement was good. Conclusions 2D-STI can be used to evaluate the left atrial function of patients with MVP.
