后路椎弓根螺钉内固定联合单枚Cage治疗37例退行性腰椎不稳症疗效分析

摘　　要：目的:探讨后路椎弓根螺钉内固定联合单枚Cage治疗退变性腰椎不稳的疗效及术式选择适应证。方法:对37例退变性腰椎不稳经过严格保守治疗无效的患者,均行后路减压单枚椎间融合器配合椎弓根螺钉内固定术。结果:37例患者经随访,患者症状均消失,腰椎高度无丢失,无断钉、断棒并发症。术后椎间融合标准按SUK标准,优29例,良8例。临床疗效根据日本JOA评分(29分法),优21例,良12例,可4例,优良率89.2%。结论对于中老年退变性腰椎不稳患者根据症状、体征、影像学检查选择经后路椎弓根螺钉内固定联合单枚Cage在治疗退变性腰椎不稳症上,植骨融合率高,症状改善明显,疗效满意。Objective To study the clinical efficacy of lumbar spine instability with Posterior decompression of lumbar fusion used single cage with pedicle screw fixation.Methods From April 2007 - october 201 1,37 cases of patients with lumbar spinal instability were opcrated with posterior fusion cage and pedicle screw fixation.Results 37 cases were followed up for 12 months-36 months（average of 18 months）,All patients was recovered,lumbar height without loss,no broken nails,broken stick complications.Fusion standard by SUK standards, Excellent in 29 cases,good in 8 cases.Clinical score based on JOA of Japan（29 rain method）, Excellent in 21 cases,good in 12 cases,mean in 6 cases,and good in 89.2 % .Conclusion Posterior decompression alone fusion cage and pedicle screw fixation in lumbar spine instability,the fusion rate is high and improving symptoms, results were satisfactory.
