联合远端蒂的下肢内侧复合皮瓣的临床应用

摘　　要：目的探讨联合远端蒂的下肢内侧复合皮瓣在足部套状撕脱伤创面修复中的应用价值。方法应用膝内侧皮瓣游离移植联合隐神经营养血管筋膜蒂皮瓣转移修复足部皮肤套状撕脱伤6例。皮瓣切取面积最大为12cm×45cm，最小为8cm×30cm。结果5例皮瓣全部成活，1例皮瓣边缘部分坏死，经换药及游离植皮后愈合。术后随访5～24个月，皮瓣外形满意，血供良好，均恢复保护性感觉，患足能负重行走，按足部疾患治疗结果评分标准评定，平均70分。结论联合远端蒂的下肢内侧复合皮瓣血供良好，皮瓣切取面积大．可恢复感觉功能，适合修复足部皮肤套状撕脱伤。Objective To introduce a new composite flap in lower limb medial of foot sleeve avulsion. Methods Grafting the medial knee flap and saphenous neurocutaneous vascular fascia flaps to repair the foot skin sleeve avulsion in 6 cases. The area of flap was 8cm ×30cm-12cm ×45cm. Results 5 flaps survived completely, and the edge of flap in 1 cases was necrosis, healed after dressing changes and skin graft. Following-up ranged from 5 to 24 months, the shape of flap was satisfactory, and blood supply was good, protective sensation restored, the foot can walk with weight-bearing. According to foot disease treatment outcome score, it obtained average 70 points. Conclusion It is suitalble that applying composite flap in lower limb medial combined with distal pedicle to repair foot sleeve avulsion, for its good blood supply, big excision area and statisfactory sensory function.
