阴道镜诊断女性下生殖道湿疣的价值

摘　　要：目的　探讨阴道镜诊断女性下生殖道湿疣的价值。方法　采用阴道镜观察 2 5 0例下生殖道湿疣的病变情况并与病理检查对照。结果　 2 5 0例湿疣患者子宫颈、阴道、外阴共发生湿疣病变 32 2处 ,其中尖锐型湿疣2 5 8例 (占 80 12 % ) ,扁平型湿疣 5 4例 (占 16 77% ) ,扁平型湿疣合并CIN、VIN10例 (占 3 10 % )。宫颈湿疣的临床检出率为 34 40 % ;阴道镜检出率为 76 80 %。阴道镜诊断尖锐型湿疣正确率为 10 0 0 0 % ,假阳性率 0 ,假阴性率 0 ,敏感度 10 0 0 0 % ,特异度 10 0 0 0 % ;扁平型湿疣正确率为 98 5 7% ,假阳性率 0 ,假阴性率 5 3 70 % ,敏感度46 30 % ,特异度 10 0 0 0 % ;扁平型湿疣合并CIN、VIN的正确率为 99 95 % ,假阳性率 0 ,假阴性率 10 0 0 % ,敏感度 90 0 0 % ,特异度 10 0 0 0 %。结论　阴道镜检查是诊断湿疣的一种方便、实用、无创伤、检出率高的较好方法。Objective To study the value of colposcopy for the diagnosis of lower female genital condyloma.Methods To study 250 cases of female genital lower condyloma with colposcopy from January 1994 to January 1998.Results There are 322 condyloma lesions in cervix,vagina and vulva in 250 cases.The condyloma acuminatum was found in 258 cases(80 12%),the flat condyloma in 54 cases (16 77%),the flat condyloma with VIN or CIN in 10 cases (3 11%).The rate of findings with clinical diagnosis for cervical condyloma was 34 40%,with colposcope was 76 80%.The rate of accuracy in the diagnosis of condyloma acuminatum with colposcopy was 100%;the false negative rate was 0,the false positive rate was 0,the sensitivity was 100%,the specificity was 100%.The rate of accuracy in diagnosis of the flat condyloma with colposcopy was 98 57%,the false positive rate was 0,the false negative rate 53 70%,the sensitivity was 46 30%,and the specificity was 100%.The rate of accuracy in diagnosis of the flat condyloma with CIN、VIN was 99 95%,the false positive rate was 0,the false negative rate was 10%,the sensitivity was 90%,and the specificity was 100%.Conclusion The diagnosis of the condyloma with colposcopy may be a feasible,practical,scatheless and highly effective new morphologic technique.
