反复自然流产的病因学研究

摘　　要：目的　探讨反复自然流产的发病原因。方法　对 2 0 8例反复自然流产患者进行妇检、盆腔B超、感染因素的检查 (支原体、衣原体、弓形体、巨细胞病毒 )、性激素水平测定 ,患者丈夫进行精液常规检查 ,患者夫妇同时检查染色体核型、ABO血型及血清IgG抗体滴度水平、抗精子抗体并进行HLA相容性研究。 结果　在病因明确的反复自然流产中 ,解剖学因素占首位 (2 0 2 % ) ,其次为感染性因素 (10 6 % ) ,ABO血型不合 (9 1% )及内分泌因素 (7 2 % )等。原因不明者占 44 2 %。结论　对反复自然流产夫妇应进行系列检查 ,尽可能确定病因学诊断 ,以免漏诊 ,同时也有利于早期针对部分病因进行处理 ,减少自然流产的发生率。Objective To detect the causes of recurrent spontaneous abortion(RSA).Methods 208 women with RSA were investigated by a series of detection including genital anatomy checkup,B-ultrasonography,sexual hormone,and pathogens of chlamydia trachomatis(CT),ureaplasma urealyticum(uu),toxoplasma(Tox) and human cytomegalovirus(CMV).The analysis of chromosomal karyotype,anti-sperm antibody,compatibility of ABO blood type and human leukocyte antigen(HLA) were given for the couples.Sperm analysis were given for their husbands simultaneously.Results Of the causes of RSA,the first is the genital abnormalities(20 2%),the second is infectious factors (10 6%),incompatibility of ABO blood type (9 1%) and endocrinologic causes (7 2%),etc.The 44 2 percent cases are of unexplained RSA.Conclusion It suggests that a series of detection must be given to affirm the causes for the couples of RSA.It could decrease the incidence of RSA by treating the risk factors early.
