摘　　要：目的 :探讨透视监控下 ,进行腰椎间盘突出症胶原酶溶解术盘外注射的穿刺方法及其安全性与准确性。方法 :为达到准确穿刺的目的 ,首先在腰椎 CT片上采用几何学方法 ,测量穿刺点位置、穿刺深度及进针角度 ,然后在透视监控下进行穿刺 ,并配合硬膜外造影以确定进针位置是否正确。结果 :70 2例全部穿刺成功 ,随访术后 3个月以上病例 12 5例 ,总有效率 92 %。无并发症和后遗症。结论 :正确掌握穿刺方法能有效地提高穿刺的安全性与准确性 ,并且疗效是肯定的。Objective:To discuss the puncture method,security and accuracy injecting collagenase outside the lumbar disc for treating lumber disc herniation under fluoroscopy monitor.Methods:To achieve the puncture accurately,the point,the angle and the depth of puncture were measured using the geomatry on lumbar CT images,then the puncture operation was performed under fluoroscopy,and the epidural myelography was done to show the location of needle.Results:The puncture operation was successful in all of 702 cases.125 cases were followed up after the operation for more 3 months,the effective rate was 92%(125 cases).There were no complications and sequelase.Conclusion:It is important to master the puncture operative method accurately that can improve much the security and accuracy of puncture a wellas the therapeutical effect for lumbar disc herniation.
