高血压患者静息心率与血压昼夜节律的相关性

摘　　要：目的探讨原发性高血压患者的静息心率(RHR)与血压昼夜节律的相关性。方法选择210例汉族中年原发性高血压患者,按RHR水平分为3组,RHR1组:RHR<70bpm(53例),RHR2组:70bpm<RHR<80bpm(92例),RHR3组:RHR≥80bpm(65例)。同时行24h动态血压检查,对3组间SBP、DBP及血压昼夜节律特征进行统计学分析。结果①24hSBP、DBP在RHR三组间比较有统计学意义(p<0.05),RHR水平与收缩压、舒张压水平均具有相关性,其相关系数分别为r=0.21,p<0.01;r=0.34,p<0.01;②随着RHR水平的增高,血压昼夜节律正常者所占的比例明显降低,昼夜节律消失的患者的比例明显增加,三组间比较有统计学差异(p<0.05)。结论 RHR增快与高血压的血压水平有相关性,对血压昼夜节律特征改变有一定影响。Objective To explore the relationship between the resting heart rate （RHR） and circadian rhythm of blood pressure in primary hypertensive patients. Methods Two hundred and ten cases with hypertension were divided into three groups： RHR1 （〈70bpm）, RHR2 （70bpm①〈RHR〈80bpm）, RHR3 （I〉 80bpm）. Their resting heart rate, 24h blood pressure were examined. The results of different groups＇ made contrast and correlation each other. Results DBP and SBP in all groups showed a statistically significant difference （p〈0.05）, and increased with RHR increasing. Correlation analysis showed that RHR was positively correlated with DBP and SBP. The proportion whose blood pressure circadian rhythm disappeared was different in different RHR groups, RHR higher, the bigger the proportion （p 〈0.05）. Conclusions RHR was associated with the level of hypertension and had an effect on circadian rhythm of blood pressure.
