摘 要:目的检测慢性阻塞性肺病(COPD)患者运动耐力与运动时通气与气体交换指标的变化,探讨其运动耐力及呼吸功能损害的机制。方法非急性发作期COPD 患者行坐式脚踏功率车运动至次极量水平,连续检测其通气与气体交换指标,并与健康对照组比较。结果 COPD组最大运动负荷(WR)减低,最大耗氧量(VO2)、最大潮气量(VT)均减低;最大通气量(VE)与对照组无差别,但通气储备量(BR)降低;两组运动中死腔量与潮气量比值(VD/VT)均显著降低,但 COPD组的降低幅度较小。结论 COPD 患者运动耐力下降,其主要因素可能是 VT的增加受限所致的通气功能损害; VD/VT比值较高所致的通气 效率降低也可能有一定影响。Objective To measure the indices of ventilation and gas exchange during exercise in patients with chronic obstructive pulmonary disease (COPD) and study the mechanism by which lung function was injured. Methods Stable stage COPD patients were performed sub-maximum exercise testing. The ventilation and gas exchange indices were measured continuously and compared with those of the health controls. Results The maximal workload (WR), maximal oxygen uptake (VO2), and maximal tidal volume (VT) in COPD group were significantly lower than those of the control group. There was no significant difference on maximal ventilation during exercise(VE) between the two groups,but the breathing reserve (BR ) in COPD group was much lower than that of the controls. VD/VT ratio decreased significantly during exercise in both groups,but the decrease in COPD patients was less than that in controls. Conclusion Exercise tolerance in COPD group is decreased,mainly due to the injured ventilatory function by which VT is limited to increase. The higher VD/VT ratio leading to lower ventilaion efficiency may also contributed to it.
