我院15年剖宫产率上升情况分析

摘　　要：目的:总结我院15年来剖宫产率上升情况,探讨如何降低剖宫产率,提高产科质量的方法。方法:回顾性分析我院自1995年1月至2009年12月间9478例孕妇临床资料,着重分析新生儿出生体重、新生儿窒息率、新生儿死亡率与剖宫产的关系。结果:⑴剖宫产率呈逐渐上升的态势,由1995年的19.21%上升至2009年的44.19%,差异有统计学意义(χ2=82.87,P<0.01)。⑵新生儿平均出生体重15年来一直稳定在3 500g左右。⑶巨大儿(wt>4 000g)在新生儿中的比例稳定在18%左右,但巨大儿剖宫产率却由1995年的26.47%猛增到2009年的69.14%,差异有统计学意义(χ2=27.26,P<0.01)。⑷随着剖宫产率的上升,新生儿窒息率由3.84%下降至3.70‰,差异有统计学意义(χ2=104.3,P<0.01)。结论:⑴新生儿出生体重偏高,提示孕妇营养过剩是15年来一直存在的问题,做好孕期营养指导,降低新生儿平均出生体重,有望降低剖宫产率。⑵巨大儿的剖宫产率的快速上升说明在一定程度内降低剖宫产率是可行的。⑶新生儿平均体重在3 500g的水平上,剖宫产率在40%左右可获得较佳的母儿结局。Objective：Summary of our hospital in the past 15 years the increasing cesarean section rate,discusses how to reduce the rate of cesarean section,to improve the quality of obstetric method.Methods：Retrospective analysis of our hospital from 1995January to2009December,9478 cases of pregnant women with clinical data,focuses on the analysis of neonatal birth weight,neonatal asphyxia,neonatal mortality and rate of cesarean section and its relation to.Results：The cesarean section rate increasing,increasing from 19.21% in 1995 to44.19% in 2009,the difference was statistically significant（χ2=82.87,P〈0.01）.The average weight of newborn babies15 years remained stable at around 3 500g.The great children（WT〉4 000g） in neonates in the proportion of stable at around 18%,but huge cesarean section rate but by 26.47% 19952009 soared to69.14%,the difference was statistically significant（χ2=27.26,P〈0.01）.with the increasing cesarean section rate,neonatal asphyxia rate reduced from 3.84% to 3.70,the difference was statistically significant（χ2=104.3,P〈0.01）.Conclusion：The neonatal birth weight high,prompting nutrition of pregnant women is in excess of15 years has been the problems,good nutrition during pregnancy,reduce the average neonatal birth weight,is expected to reduce the rate of cesarean section.The great children’s cesarean section rate rise quickly in a certain extent reduce the rate of cesarean section is feasible.The average neonatal body weight on the 3 500g level,the rate of cesarean section in 40% the left and right sides can obtain better maternal and fetal outcome.
