胱抑素C、尿Ⅳ型胶原、尿微量白蛋白联合检测在糖尿病肾病中的诊断价值

摘　　要：目的:分析糖尿病肾病早期胱抑素C(Cys-C)、尿Ⅳ型胶原(CⅣ)及尿微量白蛋白(mALB)联合检测的临床意义。方法:选取糖尿病肾病患者87例,按尿微量白蛋白排泄率(UAER)分正常蛋白尿(NA)组、微量蛋白尿(MA)组、临床蛋白尿(CDN)组及40例体检健康人群作为对照组。分析三项指标的浓度变化以及单项检测、联合检测的敏感性和特异性,行统计学分析。结果:糖尿病肾病患者胱抑素C、尿Ⅳ型胶原及尿微量白蛋白的含量均高于正常对照组;MA组与CDN组的Cys-C、尿CⅣ和mALB的含量均高于NA组;CDN组中的mALB含量明显高于MA组;三项指标联合检测诊断价值明显提高。结论:Cys-C、尿CⅣ及mALB联合检测在诊断糖尿病肾病肾损伤中有临床意义;Cys-C、mALB含量的增高反应糖尿病肾病早期肾损伤的程度。Objective：To study the clinical value and of combined testing of Cys-C,CⅣ and mALB in patients with diabetic nephropathy.Methods：According to UAER,87 patients with diabetic nephropathy were divided into NA group,MA group and CDN group,and 40 persons by medical examnation make as control group.We analyses the concentration of Cys-C,CⅣ and mALb,and whose sensitivity and specificity of the single and combine test in patients with diabetic nephropathy.Results：The concentration of Cys-C,CⅣ and mALB in patients with diabetic nephropathy were significantly higher than those in normal groups.In MA and CDN groups,the concentration of Cys-C,CⅣ and mALB were significantly higher than those in NA group.The concentration of mALB in CDN was significantly higher than that in MA group.It was more effective in the diagnosis of damage of kideny thorugh the way of comining measurement of Cys-C,CⅣ and mALB.Conclusion：The combined testing the Cys-C,CⅣ and mALB is a valuable method in diagnosis the damage kidney of the patients with diabetic nephropathy;The concentration of Cys-C,CⅣ and mALB increased with development reflect the degree of kidney demage.
