I型胶原吡啶交联终肽在评价类风湿关节炎疗效中作用

摘　　要：目的探讨I型胶原吡啶交联终肽（IcTP）对评价类风湿关节炎（RA）患者临床疗效的作用。方法80例（男20例，女60例）RA患者为2008年5月～2011年12月门诊及住院病人，年龄18-79岁，平均年龄（55．8士13）岁，病程6个月～3年，平均（15士7）月。RA诊断符合美国风湿病协会（ACR）1987年制订的类风湿关节炎诊断分类标准，所有患者治疗前一年以上未曾使用过甲氨蝶呤、来氟米特等任何治疗，观察期间给予甲氨堞呤、来氟米特二种药物联合治疗，于治疗前和治疗后6个月留取患者血清。应用ELISA法测定患者血清中ICTP水平，同时测定患者血沉（ESR）和类风湿因子（RF），并记录RA患者的疾病活动指数28（DAs28）。结果治疗前后血清中ICTP，RF差异有统计学非常显著性意义（t值分别为2．97和14．43，P〈O．01），ESR，DAS28差异显著（t值分别为2．O1和L99，P〈0．05），且发现ICTP水平与RA患者DAS28均呈明显相关（r=0．39，P=0．02），与ESR，RF无相关性。结论血清ICTP不仅能够准确地反映类风湿关节炎病情而且能较好地评价疗效。Objective Toprobeappraising clinical effects of treatment on C-terminal cross-linking telopeptide of type I collagen （ICTP） in the patients of rheumatoid arthritis （RA）. Methods 80 patients were treated with methotrexate and leflunomide. The serum level of ICTP was determined in those patients by radioimmunoassay, and C-terminal cross-linking telopeptide of type I collagen（ICTP）, erythrocyte sedimentation rate（ESR）, rheumatoid factor（RF） and disease activity score 28 were also reeored at baseline and at months 6. Results Serum ICTP,ESR,RF and DAS28 levels of RA were signfically reduced after the therapy（ P〈0. 05, P〈0. 01）. Moreover, the levels of ICTP were positively correlated with DAS28 （r= 0. 39, P= 0. 02）. Conclusion The serum level of ICTP in the patients of rheumatoid arthritis can not only exactly reflect patient＇s condition but also better evaluate curative effect.
