摘 要:目的:探讨经皮激光椎间盘减压术(PLDD)治疗腰椎间盘突出症治疗效果,临床分度(Ⅰ、Ⅱ、Ⅲ度)与激光照射量的相关性,研究出相关激光照射量的大小。方法:收集46例腰椎间盘突出症患者,腰4~5椎间盘突出34例;腰5骶1和腰4~5同时突出8例;腰5骶1和腰4~5同时突出并汽化5例。C臂X线机透视下将激光光纤沿穿刺针插入椎间盘髓核中进行汽化。依据临床分度分成Ⅰ、Ⅱ、Ⅲ度,每一度为一组,并对46例患者进行激光照射量的统计,每两组间激光照射剂量比较采用SNK-q检验。结果:纳入研究的46例腰椎间盘突出症患者均进入结果分析:临床分度分成Ⅰ、Ⅱ、Ⅲ度,每两组间激光照射剂量无明显差异(P>0.05)。采用的Macnab标准评定疗效:优31例,良6例,可7例,差2例,优良率为80%。结论:PLDD腰椎间盘突出症激光照射剂量在500~710 J之间,不能因为患者临床症状重而选择较大剂量的激光,把握好激光照射量会取得满意的治疗效果。Objective: To investigate the skin laser disc decompression (PLDD) treatment prolapse of lumbar intervertebral disc treatment effect, clinieal dividing (Ⅰ、Ⅱ、Ⅲ degree) and the amount of the laser light eorrelation, study the correlation to the quantity of the laser light. Methods: 46 eases of patients with lumbar intervertebral disc herniation, waist 4-5 interverbebral dise highlight 34 eases. Waist 5 sl and waist 4-5 and 8 eases of prominent; Waist 5 sl and waist 4-5 also highlights and vaporization 5 eases. C arm X-ray machine under optical fiber laser perspective along the needle insert disc nucleus pulpous in vaporized on impact. Based on clinical dividing into Ⅰ、Ⅱ、Ⅲ degrees, every once as a group, and of 46 patients of laser irradiation amount of statisties, between the two groups eaeh laser radiation dose compares the SNK-q inspection. Results: In the 46 eases of lumbar intervertebral dise herniation into results all patients: clinical dividing into Ⅰ、Ⅱ、Ⅲ degrees, between the two groups each laser radiation doses no significant difference (P 〉 0.05). The standard of Maenab group the efficacy: optimal 31 eases "good" in 6, can be in 7 eases, poor in 2 eases, and was 80%.Conclusion: PLDD prolapse of lumbar intervertebral disc laser radiation doses in 500-710 between J; not because the clinical symptoms and select bigger doses of heavy laser, grasp the laser energy may obtain satisfactory results.
