前列地尔脂球载体制剂联合洛沙坦、雷公藤多甙对2型糖尿病肾病肾脏血流动力学的干预作用

摘　　要：目的探讨前列地尔脂球载体制剂联合洛沙坦、雷公藤多甙对2型糖尿病肾病肾脏血流动力学的干预作用。方法选择2型糖尿病早期糖尿病肾病患者82例,随机分为治疗组和对照组,用彩色多普勒血流显像分别测定治疗组和对照组用药前后肾主动脉(MRA)、段动脉(SRA)、叶间动脉(IRA)变化情况;观察治疗前后24 h尿蛋白定量(Upr)、糖化血红蛋白(HbA1c),尿肌酐(Ucr),血肌酐(Scr),肌酐清除率(Ccr);空腹血糖(FBS)、血尿素氮(Bun)变化。结果与对照组比较,治疗组肾血流动力学,Bun、Scr、Ccr、Upr/24 h均明显改善,经统计学处理有显著性差异(P<0.01)。结论前列地尔脂球载体制剂联合洛沙坦、雷公藤多甙治疗2型糖尿病肾病,可显著降低肾动脉血流指数,可改变糖尿病肾病患者肾脏的高滤过高灌注状态,改善肾功能,值得临床推广应用。Objective To investigate the effects of lipo prostaglandin combined with losartan, and tripterygium wilfordii polyglycosidium on renal hemodynamics of type 2 diabetic nephropathy. Methods A total of 82 patients with early diabetic nephropathy of type 2 diabetes were divided into the treatment group and the control group. The change of the renal artery （MRA）, the segmental renal artery （SRA） and the interlobular artery （IRA） were observed before and after the treatment by using color doppler uhrasonography; Upr, HbA1 c, Ucr, Scr, Ccr, FBS, Bun were observed before and after treatment. Results Compared with the control group, the renal hemodynamics, Bun, Scr, Ccr, Upr/24h of the experimental group were significantly improved. The difference was significant （ P 〈 0.01 ）. Conclusion The treatment of type 2 diabetic nephropathy with lipo prostaglandin combined with losartan, and tripterygium wilfordii polyglycosidium can notably reduce the renal artery blood flow, change the condition of the diabetic nephropathy＇ s hyperfihration and hypertransfusion, improve the renal function, so it is worthy of further promotion and studying in clinic.
