藏族成人高值血压与心血管病危险因素和心功能的相关性

摘　　要：目的:分析四川省松潘县藏族成人高值血压分布特点,探讨高值血压与心血管病危险因素和心功能的相关性,为藏族成人健康管理提供理论依据。方法:采用流行病学横断面的研究方法,对四川省松潘县18～74岁1024名藏族成人进行了多阶段、分层、非等比例、整群抽样调查。结果:藏族成人高值血压占43.1%,高值收缩压占31.0%,高值舒张压占19.6%。多因素线性回归分析显示:收缩压高值与超重、肥胖、脉压、农民职业正相关。多因素logistic回归显示:收缩压高值与年龄、肥胖、心率、脉压、大学学历正相关,与女性负相关。舒张压高值与年龄、腰围、高中学历正相关;脉压与舒张压负相关。结论:四川省松潘县18～74岁藏族成人高值血压比例较高,高值血压与心血管病危险因素和心功能指标有关。Objective：To explore the relations of high-normal blood pressure （BP） to cardiovascular risk factors and the cardiac function based on analyzing the distributions of high-nurmal BP in Tibetan adults. Methods： A cross sectional study was carried out in Songpan county, Sichuan province. The multi-phase, stratified, unequal proportional and cluster sampling was conducted to investigate 1024 Tibetan adults aged 18-74 years in 2007. Results：In Songpan county the distributions of the high-normal BP, high-normal systolic BP （SBP）, high-normal diastolic BP （DBP）, were 43. 1%, 31.0% and 19.6%, respectively. High-normal BP was significantly related to many risk factors of cardiovascular disease. Multivariable regression analysis showed that high-normal SBP was positively related to over weight, obesity, pulse pressure and farmer occupation, respectively. Muhivariable logistic analysis displayed that the high-normal SBP was positively related to age, obesity, heart rate, pulse pressure and college education, but negative correlated with female gender in contrast to male. The high-normal DBP was positively related to age, waist circumference, and high school compared with primary school, while was negatively related to pulse pressure. Compared with those with normal BP, in the patients with high-normal BP there was an increase in the mean systemic vascular resistance （P〈0.05）. Conclusion： High-normal BP is common in Tibetan adults aged 18-74 years, which is independently associated with cardiovascular risk factors and cardiac function.
