急性心肌梗死患者临床特征及PCI治疗效果的性别差异

摘　　要：目的探讨不同性别急性心肌梗死患者的临床特征及经皮冠状动脉介入术(PCI)的治疗效果和预后的差异。方法选择2009年1月-2011年9月因急性心肌梗死住院并行PCI的285例患者,根据性别分为女性组和男性组,对比分析其临床特征及治疗效果和预后的差异。结果与男性组比较,女性组患者年龄大,1周内口服阿司匹林、合并高血压、糖尿病、肾功能不全、伴心力衰竭、Killip分级>1级的比率高,LVEF值低;而男性吸烟多于女性冠脉造影结果显示,女性患者冠脉三支病变、非靶血管狭窄>70%、术后TIMI血流<3级、并发心力衰竭、肺炎等比率高于男性;女性患者住院期间病死率显著高于男性患者(18.4%vs 3.0%,P<0.01)。结论女性发生AMI的时间较男性晚,但其危险因素、合并症、血管严重程度、并发症、住院病死率等均较男性高。Objective To investigate the clinical features, treatment and prognosis difference of patients with acute myocardial infarction in different gender undergoing PCI. Methods 285 patients with acute myocardial infarction admitted to our hospital from January 2009 to September 2011 were enrolled. They were divided into the female group and male group ac- cording to gender, and analyzed the differences of their clinical features and outcomes. Results The female patients have old age, high proportion of oral aspirin within a week, and high ratio of hypertension, diabetes, renal insufficiency, more likely to have heart failure, Killip class 〉 l , and low LVEF; but men occurred with smoke more than women. Coronary angiography showed that female patients have higher ratio of coronary three vessel disease , non-target vessel stenosis 〉 70% , postopera- tive TIMI blood flow 〈 3 than men, and more heart failure, pneumonia ; hospital fatality of female patients WAs significantly higher than male patients （ 18.4% vs. 3.0% , P 〈 0.01 ）. Conclusion The oecurrence of AMI in women is late, but its risk factors, complications, blood vessels stenosis severity, hospital fatality rate are higher than men.
