摘 要:目的:检测初治和复治结核病患者外周血NK细胞数量和胞内杀伤分子的变化。方法:取健康人(112名)和结核病患者145例(初治105例,复治40例)抗凝外周全血标本,加荧光标记抗CD3、CD56、CD8、颗粒酶和穿孔素抗体进行标记,进行流式细胞术检测。结果:初治肺结核患者外周血NK细胞的比例(16.95±8.27)%显著低于健康组(19.99±9.28)%(P<0.01)。健康组外周血NK细胞绝对值计数为(44.32±26.69)×104/ml,结核初治组为(33.31±21.44)×104/ml,复治组为(36.8±24.97)×104/ml,结核初治组明显低于健康组(P<0.01)。结核病患者NK细胞中表达颗粒溶解素(85.78±11.63)%,穿孔素(82.88±17.05)%,颗粒酶(79.07±15.64)%,健康组(87.31±10.34)%、(82.85±13.03)%、(88.04±7.27)%,2组颗粒溶解素和穿孔素差异均无统计学意义(P>0.05),而颗粒酶差异有统计学意义(P<0.01)。结论:初治组结核病患者外周血NK细胞数量明显降低,但NK细胞中颗粒溶解素、穿孔素的表达和健康组差异均无统计学意义。Objective: To detect the changes of the number of NK cells and intraceUular killing molecules in peripheral blood ofpatients with pulmonary tuberculosis. Methods:Percentages of NK cells in peripheral whole blood samples from 145 pulmonary TB patients( 105 first-treated pulmonary TB and 40 retreated pulmonary TB) and 112 healthy controls were collected, labelled with antiCD3, CD56, CD8, granulysin, perforin and granzyme antibody, and analyzed by flow cytometry. Results: The percentage of NK cells in first-treated pulmonary TB patients was ( 16.95±8.27) %, which was significantly lower than that in healthy controls ( 19.99 ± 9.28 ) % (P 〈 0. 01 ). Absolute value of NK cells in first -treated pulmonary TB patients was (33.31 ±21.44) × 10^4/ml, which was significantly lower than (hat in healthy controls (44.32 ± 26.69 ) × 10^4/ml) ( P 〈 0.05 ). Absolute value of NK cells in retreated pulmonary TB patients was (36.8 ± 24.97 )× 10^4/ml. Granulysin [ ( 85.78 ± 11.63 ) % vs ( 87.31 ± 10.34) % ], perforin [ ( 82.88± 17.05 )% vs (82.85 ±13.03 )% ] in NK cells had no difference bewteen pulmonary rib patients and healthy controls(P 〉 0.05 ), but the difference of granzyme [ (79.07 ± 15.64 ) % vs ( 88.04 ± 7.27 ) % ] was significant ( P 〈 0.01 ). Conclusions: The number of NK cells in peripheral whole blood of first-treated pulmonary TB patients NK was significantly decreased. However, the expressions of granulysin and pefforin in NK cells had no difference between pulmonary TB patients and healthy controls.
