高频彩色多普勒超声对腘窝包块的诊断价值

摘　　要：目的探讨高频彩色多普勒超声对腘窝包块的诊断价值。方法对以腘窝包块就诊的患者166例应用高频彩色多普勒超声对腘窝肿块进行检查,观察其声像结果图特征,结合病史,预测腘窝肿块良恶性,并与经手术、穿刺病理证实的结果进行对照分析。结果 166例腘窝包块以囊性肿物为主(100例),而囊性肿物以腘窝囊肿为主,高频彩色多普勒超声诊断59例腘窝囊肿(诊断符合率达100%),对囊性肿物的诊断符合率为99%;实质性肿物以脂肪瘤为主,对实质性肿物的诊断符合率为74%;囊实混合性包块以血管瘤为主,对囊实混合性包块的诊断符合率为82.1%,对腘窝包块总的诊断符合率为91.0%。结论利用高频彩色多普勒超声不但能清晰显示腘窝包块的内部结构、周围彼邻关系,而且还能显示病变的血供情况,对诊断、鉴别诊断及临床选择治疗方式有重要价值。Objective To investigate the value of high-frequency color Doppler ultrasound in the diagnosis of popliteal masses. Methods One hundred and sixty-six patients diagnosed as popliteal masses were examined by high-frequency color Doppler ultrasound. We observed the audio-visual map characteristics, predicted popliteal tu- mors as benign or malignant combined with the history of disease, and comparatively analyze the results confirmed by surgery and biopsy. Results Of the 166 cases, the majority of popliteal masses were cystic masses （100 cases）, and cystic mass were mainly found to be popliteal cyst. High-frequency color Doppler ultrasound showed 59 cases ofpop- liteal cysts, with the diagnostic accuracy of 100%. The diagnostic accuracy of cystic masses was 99%. Substantial tu- mors were mainly lipoma, with the diagnostic accuracy for substantial tumor of 74%. The majority of mixed solid and cystic masses were hemangioma, with the diagnostic accuracy for mixed solid and cystic masses of 82.1%. The total diagnostic accurary of popliteal masses was 91%. Conclusion High-frequency color Doppler ultrasound can clearly display the internal structure of popliteal masses around the neighborhood relations and the blood supply of lesions, which plays an important role in the diagnosis and differential diagnosis of popliteal masses as well as the selection of treatments.
