摘 要:[目的]评价初诊活动性肺结核患者痰模型涂片法检测抗酸杆菌的应用效果。[方法]2010年1月至2012年6月,在泰安市的宁阳县、新泰市和泰山区,分别收集初诊活动性肺结核患者(每例当日即时痰、夜间痰、次日晨痰各1份)的痰标本780份(应用一组)、816份(应用二组)、1 089份(研究组),应用模型法涂片后镜检抗酸杆菌。[结果]痰标本涂片抗酸杆菌阳性检出率,应用一组、应用二组、研究组分别为29.74%、28.92%、31.68%(P>0.05);涂片痰细胞合格率分别为94.10%、94.98%、94.21%,痰膜大小合格率分别为98.72%、99.14%、98.99%,厚薄合格率分别为95.64%、95.22%、96.14%、染色合格率分别为93.72%、95.10%、94.49%,痰膜脱落合格率分别为97.18%、98.28%、97.43%,差异均无统计学意义(P>0.05)。从3组分别抽检50、55、80张痰涂片,原始结果与复检结果的阳性符合率、阴性符合率均为100.00%。[结论]肺结核病人痰模型法涂片后检测抗酸杆菌,制片质量较高,检查结果较为准确。[Objective]To evaluate the application value of the sputum membrane model detecting mycobaeterium tuber- culosis in newly diagnosed active pulmonary tuberculosis patients. [Methods]A total of 780 sputum specimens (the first application group), 816 copies(the second application group), 1 089 copies (the study group)were detected for mycobacterium by the sputum membrane model,which were collected from newly diagnosed active pulmonary tuberculosis patients (day instant sputum,night sputum,next morning sputum) in Ningyang, Xintai and Taishan districts of Taian city, from January 2010 to June 2012; [Results]The first study group, the second study group and the experimental group were detec- ted for mycobacterium tuberculosis,and the positive rate was 29.74 %, 28.92 % and 31.68 % ( P 〉0.05), the cells in spu- tum smear ( 94.0%, 94.98%, 94.21%), sputum membrane size ( 98.72%, 99.14%, 98.99%), thickness ( 95.64%, 95.22%, 96.14%), dyeing (93.72%, 95.10%, 94.49%), sputum membrane shedding pass rate ( 97.18%, 98.28%, 97.43%). The five quality indicators differences of the groups were not statistically significant( P〉0.05). The submis- sion of 50,55 and 80 sputum samples original and recheck results of the positive coincidence rate, and the negative coinci- dence rate were 100.00% ,which were from the three groups. [Conclusion]The sputum membrane model detecting myco- bacterium tuberculosis improves the quality and inspection results accuracy.
