早期乳腺癌保乳术加放射治疗与改良根治术患者的相关心理问题的对照研究

摘　　要：目的探讨早期乳腺癌保乳术加放射治疗与改良根治术后1年患者的相关心理问题。方法选择40例早期乳腺癌保乳术加放射治疗后1年的患者为研究组,选择同期22例早期乳腺癌改良根治术后1年的患者做对照组,研究其相关心理问题并2组对照。结果研究组焦虑和抑郁的发生率分别为40.0%、45.0%,对照组焦虑和抑郁的发生率分别为45.5%、63.6%,2组比较差异无统计学意义(均P>0.05);研究组形体不满意、性吸引力下降、性生活兴趣度下降的发生率分别为17.5%、35.0%、37.5%,对照组形体不满意、性吸引下降、性生活兴趣度下降的发生率分别为95.5%、90.9%、86.4%,2组比较差异有统计学意义(均P<0.001)。结论早期乳腺癌保乳术加放射治疗与改良根治术能使患者心理更健康,术后生活质量更高。Objective To investigate the psychological problems of patients 1 year after early breast conserving surgery plus radiation therapy or modified radical mastectomy.Methods Forty patients who underwent early breast conserving surgery and radiation therapy were selected as the study group,and 22 patients who underwent modified radical mastectomy were selected as the control group.The psychological problems were compared between the two groups 1 year after treatment.Results The incidences of anxiety and depression were,respectively,40.0% and 45.0% in study group,and 45.5% and 63.6% in control group.There were no significant differences between the two groups（P〉0.05）.The incidences of body dissatisfaction,sexual attraction reduction and sexual interest decline were,respectively,17.5%,35.0% and 37.5% in study group,and 95.5%,90.9% and 86.4% in control group.There were significant differences between the two groups（P〈0.001）.Conclusion Patients who underwent conserving surgery plus radiation therapy have better mental health and higher quality of life than patients undergoing modified radical mastectomy.
