摘 要:目的探讨初诊2型糖尿病(T2DM)患者胰腺脂质沉积与脂代谢紊乱的关系。方法收集T2DM患者64例和对照组60例,均接受上腹部CT平扫,计算脾胰CT差值,评估胰腺脂质沉积程度;设定脾脏CT值-胰腺CT值差值>5 HU为脂肪胰组,≤5 HU为非脂肪胰组。将T2DM患者再分为单纯T2DM组(20例),合并脂肪胰组(44例)。测定各组血浆三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)含量评估脂代谢状况。结果脾胰CT差值T2DM组为(11.7±8.9)Hu较对照组的(8.9±6.9)Hu有所增加;两组TG、HDL差异有统计学意义(P<0.05)。偏相关分析表明,脾胰CT差值与TG正相关(r=0.525),与HDL负相关(r=-0.575)。结论胰腺脂质沉积程度与脂代谢密切相关,可能参与T2DM患者代谢紊乱的发生、发展。Objective To investigate the relationship between fat deposition in pancreas in newly diagnosed type 2 diabetes mellitus (T2DM) patients with lipid metabolism disorders. Methods Sixty-four newly diagnosed T2DM patients and sixty healthy subjects served as control were selected with the average age of 67.1 ± 6.8years old. All subjects received upper abdominal CT scan and Hounsfield units (Hu) were measured, the difference between the mean values of spleen and pancreas were calculated and the extent of fat deposition in the pancreas was assessed. According to the degree of the differences,T2DM patients further divided into groups of simply T2DM (20 cases) and T2DM combined with fat pancreas (44 cases). The contents of plasma triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL) ,low density lipoprotein cholesterol (LDL) of all the patients were determinated. Results Compared with the control ones,T2DM patients had significant higher levels at the difference be- tween the mean values of spleen and pancreas. The difference of TG and HDL between the two groups was also signifi- cant ( P 〈 0.05). The partial correlation analysis showed that the difference between the mean values of spleen and pancreas was positively correlated with TG ( r = 0.525 ), and negatively correlated with HDL ( r = - 0. 575 ). Conclu- sion The lipid deposition in pancreas had a close relationship with lipid metabolism and it might be involved in the occurrence and development of metabolic disorders in patients with T2DM.
