摘 要:目的探讨双歧杆菌四联活菌对极低出生体重儿T细胞亚群和免疫球蛋白免疫功能的影响,以及对喂养不耐受的预防效果。方法选取符合标准的极低出生体重儿60例,采用随机数字的方法分为观察组和对照组,每组各30例,二者均给予常规治疗,观察组加用口服双歧杆菌四联活菌,疗程结束比较二者外周血免疫学指标和喂养不耐受发生率。结果观察组喂养不耐受发生率为16.67%,明显低于对照组的43.33%,差异有统计学意义(P〈0.05)。疗程结束后,观察组外周血CD4+细胞比例、CD4+/CD8+比值均高于对照组,差异有统计学意义(P〈0.05);而CD3+和CD8+细胞比例,两组间比较差异无统计学意义(P〉0.05);观察组IgA水平明显高于对照组,差异有统计学意义(P〈0.05),但是两组IgG和IgM含量比较,差异无统计学意义(P〉0.05)。结论极低出生体重儿应用双歧杆菌四联活菌,可以促进其免疫功能的成熟和发展,有效预防喂养不耐受的发生,值得临床推广应用。Objective To investigate the Bifidobacterium tetravaccine' s effect on T-cell subsets and immunoglobulin immune function very low birth weight infants, as well as the preventive effect of feeding intolerance. Methods 60 cases of very low birth weight infants were selected, using a random number method, they were divided into the observation group and the control group, 30 cases in each group, two were given conventional treatment, the observation group was combined with oral tetralogy of viable Bifidobacterium, by the end of the treatment their peripheral blood immunological index and feeding intolerance rate were compared. Results The observed group feeding intolerance incidence rate was16.67%, significantly lower than the control group, the difference was statistically significant ( 43.33%, P〈0.05 ) .After the end of treatment, the observation group of peripheral blood CD4+cells, the proportion of CD4+/CD8+ratios were higher than those in the control group, the difference has statistical significance (P〈0.05) ; while the CD3+ and CD8+ cell ratio, between the two groups compare difference not to have statistical significance ( P〉0.05 ) ; IgA levels were significantly higher in observation group the control group, the difference was significant ( P〈0.05 ) , but the two groups of IgG and IgM content, the difference was not statistically significant (P〉0.05 ) . Conclusion The application of Bifidobacterium tetravaccine to low birth weight infants can promote the immune function of maturity and help for an effective prevention of feeding intolerance, and is worthy of clinical application.
