摘 要:目的探讨内源性硫化氢(H2S)与高原地区慢性肺心病肺动脉高压的相关性。方法高原地区慢性肺心病稳定期患者72例和健康对照组40例,分别测定血浆H2S、内皮素-1(ET-1)、平均肺动脉压(mPAP)、动脉血氧分压(PaO2)和二氧化碳分压(PaCO2)。结果肺心病稳定期患者血浆H2S、PaO2水平显著低于健康对照组(均P<0.01),而血浆ET-1、mPAP和PaCO2水平显著高于健康对照组(均P<0.01)。肺心病组患者血浆H2S水平与ET-1、mPAP、PaCO2呈显著负相关(分别为r=-0.718、-0.692、-0.556,均P<0.01),与PaO2呈显著正相关(r=0.583,P<0.01)。结论内源性H2S可能参与了高原肺心病患者肺动脉高压的发生发展,血浆H2S作为一种无创指标监测PAH的严重程度和预后具有一定意义。Objective To observe the correlation between endogenous hydrogen sulfide (H2 S) and pulmonary arteries hyperten- sion (PAH) in patients with chronic cor pulmonale at high altitude (HACCP). Methods The levels of plasma H2 S, endothelin-1 ( ET- 1 ) , partial pressure of artery blood 02 ( PaO2 ) and partial pressure of in artery blood CO2 ( PaCO2 ) were measured in 72 patients with sta- ble HACCP and 40 health subjects. Results Levels of plasma H2 S and PaO2 were significantly lower in patients with HACCP than those in health subjects ( all P 〈0. O1 ), and levels of plasma ET-1, mPAP and PaCO2 were significantly higher in patients with HACCP than those in health subjects (all P 〈 0. 01 ). In patients with HACCP, the plasma H2 S was negatively correlated with plasma ET-1, mPAP and PaCO2 ( r = - 0. 718, - 0. 692, - 0. 556, respectively, all P 〈 0.01 ) , and was positively correlated with PaO2 ( r = 0. 583, P 〈 0. 01 ). Conclision Endogenous H2S may be involved in the formation and development of PAH in HACCP and may be a noninvasive marker of PAH severity and prognosis of those patients.
