摘  要:目的对临床疑似发育性髋关节异常(DDH)的患儿进行超声检查,使其获得早期诊断和治疗。方法对我院439例临床疑似DDH患儿行超声检查,通过测量双侧髋关节α、β角度,观察冠状面髋臼形态、股骨头位置,进行髋关节Graf分型。结果临床疑似DDH439患儿(878个髋关节)中,Ⅰ型782个(89.07%),Ⅱa型71个(8.09%),Ⅱb型17个(1.94%),Ⅱc型0个(0%),Ⅱd型3个(0.34%),Ⅲ型4个(0.46%),Ⅳ型1个(0.11%)。其中正常髋关节(Ⅰ型)占89.07%(782/878),欠成熟髋关节(Ⅱa型)占8.09%(71/878),异常髋关节(Ⅱb~Ⅳ型)占2.85%(25/878)。结论髋关节超声筛查能使DDH患儿得到早期诊断和治疗,减低晚期DDH的初诊发生率,从而降低晚期DDH的治疗困难和费用。Objective To study the value of ultrasound examination in improving the early diagnosis and treatment of suspected developmental dysplasia of the hip (DDH) in infants. Methods Ultrasound examination was performed in 439 cases of clinically suspected DDH, the α and β angle of bilateral hips were measured, the acetabular morphology and the femoral head position were observed followed by Graf classification. Results Among all the 439 clinically suspected cases (878 hips), 782 (89.07%)cases were type [ ,71 (8.09%) cases were type Ⅱ a, 17 ( 1.94% ) cases were type Ⅱ b, none was type Ⅱ c, 3(0.34%) were type Ⅱd, 4 (0.46%) were type Ⅲ, 1 (0.11%) were type Ⅳ. 89.07% (782/878) were normal hip joints (type Ⅰ ), 8. 09% (71/878) were less mature hip joints(type Ⅱ a), 2. 85% (25/878) were abnormal hip joints (type Ⅱ b-Ⅳ ). Conclusion Uhrasound screening improved early diagnosis and treatment for infants with DDH, and reduced the incidence of late DDH, which could also be beneficial to decrease the expense on late DDH treatment.
