甘精胰岛素联合那格列奈控制波动性高血糖的疗效观察

摘　　要：目的观察甘精胰岛素联合那格列奈对老年2型糖尿病患者血糖波动的疗效及安全性。方法根据动态血糖监测系统（CGMS）结果，选择波动性高血糖患者60例，在控制饮食基础上给予甘精胰岛素联合那格列奈治疗12周，观察治疗前后患者的血糖波动变化。结果患者的糖化血红蛋白（HbA1c）、平均血糖水平（MBG）、平均血糖水平标准差（SDBG）、平均血糖波动幅度（MAGE）、日间血糖平均绝对差（MODD）分别由治疗前的（8．39±0．74）％、（11．04±2．18）mmo]／L、（2．35±0．54）mmol／L、（5．84±1．32）mmol／L、（1．92±0．63）mmol／L下降至治疗后的（7．17±0．47）％、（8．06±2．05）mmol／L、（1．51±0．32）mmol／L、（4．30±1．06）mmol／L和（1．24±0．47）mmol／L，差异均有统计学意义（P〈0．05，P〈0．01）。本组无低血糖发作，无其他不良反应发生。结论甘精胰岛素联合那格列奈可有效控制老年2型糖尿病患者血糖水平及波动幅度，且安全性高。Objective To study on the efficacy and safety of insulin glargine combined with nateglinide in elderly patients newly diagnosed type 2 diabetic mellitus with fluctuating high glucose. Methods Patients with fluctuating high glucose were given insulin glargine combined with nateglirlide based on diet control for twelve weeks, and their fluctuation of blood glucose was observed before and after the treatment. Results Patients with HbAlc, MBG, SDBG, MAGE and MODD by respectively before treatment （8.39 ± 0.74） %, （ 11.04 ± 2.18 ） mmol/L, （2.35 ± 0.54） mmol/L, （5.84 ± 1.32） mmol/L, （ 1.92 ± 0.63 ） mmol/L fell to after treatment （7.17 ± 0.47 ） %, （8.06 ± 2.05 ） mmol/L, （ 1.51 ± 0.32 ） mmol/L, （4.30 ± 1.06 ） mmol/L, （ 1.24 ± 0.47 ） mmol/L, differences were statistically significant （ P 〈 0.05 or P 〈 0.01 ）. Conclusion Insulin glargine combined with nateglinide can effectively control the level and fluctuation of blood glucose in elderly T2DM patients, with higher safety.
