颈动脉内中膜厚度检测在老年2型糖尿病中的意义及其影响因素

摘　　要：目的探讨老年2型糖尿病患者颈动脉内中膜厚度(carotid intima-media thickness,CIMT)及其与糖化血红蛋白(HbA1c)、尿酸(UA)、低密度脂蛋白胆固醇(LDL-C)的相关性。方法选择60例老年2型糖尿病患者,其中CIMT增厚组32例,CIMT正常组28例,另取40名健康老年人作为对照组。分别测定CIMT、HbA1c、UA及LDL-C行统计学分析。结果 2型糖尿病CIMT、HbA1c、UA、LDL-C明显高于对照组,2型糖尿病患者中CIMT增厚组较无CIMT增厚组HbA1c、UA升高。相关性分析显示CIMT与HbA1c、UA呈正相关。结论 HbA1c、UA是影响颈动脉CIMT的相关因素,检测这些指标有助于尽早发现糖尿病大血管病变,并协助预测心脑血管疾病发生。Objective To discuss the correlation between carotid intima-media thickness （CIMT）and glyeosylated hemoglobin（HbAlc）, uric aeid（UA）and low density lipoprotein cholesterol（LDL-C）in elderly patients with type 2 diabetes mellitus（T2DM）. Methods 60 elderly patients with T2DM,including 3Z with thickened CIMT and 28 with non-thickened CIMT, were choosen. Control group consisted of 40 healthy old men. Levels of CIMT, HbAle,UA and LDL-C were examined for statistical analysis. Results Levels of CIMT, HbAIc,UA and LDL-C in T2DM group were higher than that in control group. Levels of HbAlc and UA were higher in thickened-CIMT group than that in non-thickened-CIMT group. CIMT was positively correlated to levels of HbAlc and UA. Conclusions Observation of HbAlc and UA, which are significantly associated with CIMT, will help to detect macrovascular complications in T2DM and facilitate the prediction of cardiovascular and cerebrovascular events.
