冠脉介入术后再狭窄原因分析及对策

摘　　要：目的:探讨冠脉介入术后发生再狭窄的相关因素以及相关对策。方法:选取冠状动脉介入治疗患者577例,对其发放问卷调查了解其性别、年龄、既往史和个人史。根据其是否有急性心肌梗死、糖尿病以及再发心绞痛而分别分为两组,使其同一因素的两组有可比性。比较不同因素两组各自发生再狭窄的几率。结果:有急性心梗、糖尿病和再发心绞痛患者发生再狭窄的几率分别为12.88%、17.58%和12.05%,明显高于无急性心梗、糖尿病和再发心绞痛患者。各因素内两组比较差异明显,有统计学意义(P<0.05)。结论:冠脉介入术后再狭窄与多种因素密切相关,对于存在相关危险因素的患者应格外注意。可通过血管内超声引导下进行介入治疗;介入治疗成功后可通过药物预防再狭窄的发生;可行球囊扩张、药物洗脱支架置入支架,可通过搭桥手术进行治疗。Objective： The objective of to make a research of the reasons and solutions of Percutaneous Con）nary Intervention Rest- enosis. Methods：577 patients of coronary ar- tery intervention therapy are chosen. We is- sue questionnaires to them for investigation of their age, sex, past medical history and personal history. We divide them into two groups according to whether they have suf- fered from Acute Myocardium Infarction,Di- abetes Mellitus and Recurrence Angina Pec- torts so that patients with same factors can be cnmpared. We compare the Restenosis - sufi%ring rate of tile two groups with different factors. Results： Restenosis - suffering rate of Patients with Acute Myocardium Infarc- tion,Diabetes Mellitus and Recurrence An- gina t^ectoris are 12. 88% , 17. 58% and 12. 05% separately, which is obviously higher than the rate of patients without those diseases. The two groups have great differ- ences for all the factors and the results have statistical significanees（ P 〈 0.05 ）. Conclu-sion ： Percutaneous Coronary Intervention Restenosis is closely related to many fac- tors. Patients with relevant risk factors shall pay special attention to these factors. The in- terventional therapy can be done through the intra - vascular ultrasound, after that, pa- tients can take precautions against restenosis through chemoprophylaxis. The patients can be treated through balloon dilatation, drug e- luting stent and the bypass surgery.
