摘 要:目的探讨中耳加压对豚鼠膜迷路积水的治疗作用及其机制。方法选择50只豚鼠,随机分为对照组、加压对照组、积水4周组、积水8周组、加压组,每组10只。积水4周组、积水8周组、加压组采用腹腔注射醛固酮制备膜迷路积水动物模型。加压治疗豚鼠在第5周起在压力舱内进行加压治疗,共3周。分别观察各组耳蜗形态和功能,并用免疫组化SP法及Western blotting检测各组耳蜗水通道蛋白1(AQP1)的表达。结果对照组及加压对照组无膜迷路积水,成模各组均出现不同程度膜迷路积水,积水8周组蜗管与蜗管加前庭阶的面积比较积水4周组、加压组明显升高(P均<0.01)。各组实验前听性脑干反应(ABR)阈值比较无统计学意义;造模成功后,各成模组ABR阈值较实验前明显升高(P均<0.01),且积水8周组较其余各组明显升高(P均<0.05)。AQP1表达主要在螺旋韧带基底部、Corti's器基底膜等处,积水8周组AQP1表达较积水4周组、加压组表达减少(P均<0.01)。结论加压治疗可显著减轻豚鼠膜迷路积水且可改善耳蜗功能,其作用机制可能与AQP1表达改变有关。Objective To study the mechanism of endolymphatic hydrops and hyperbaric therapy on endolymphatic hydrops. Methods Fifty guinea pigs were divided into five groups. The animals in study were injected aldosterone O. 1 mg/( 100 g · d) in the first 5 days, and the pressure group received hyperbaric oxygen theapy from the fifth week to seven week, using Western blot and SP method to watch the expression of AQP1 in the inner ears of all guinea pigs. Rusults Average hearing threshold of auditory brainstem response (ABR) in the pressure group was lower than that of the study group of 8 weeks ( P 〈 0. O1 ), and in the study group of 8 weeks was higher than 4 weeks group and the control group. En- dolymphatic hydrops in the pressure groupand in the study group of 4 weeks was lighter than that of the study group of 8 weeks (P 〈0.01 ). AQP1 were found in the basal parts of the spiral ligament, Corti's organ. The expression of AQP1 was weaker in the study group of 8 weeks than that in those study group of 4 weeks and in the control one (P 〈 O. O1 ). The ex pression of AQP1 in the pressure group was higher than that in the study group of 8 weeks ( P 〈 0.01 ). Conclusions Hy perbaric therapy can significantly suppress the development of endolymphatic hydrops and improve cochlear function in guinea pigs, its mechanism may be related to changes in expression auapofin.
