清化瘀毒方治疗湿热瘀毒型酒精性肝硬化临床研究

摘　　要：目的:观察清化瘀毒方治疗酒精性肝硬化的临床疗效。方法:将60例符合酒精性肝硬化诊断标准及中医辨证属湿热瘀毒型患者按随机数字表法分为治疗组和对照组,治疗组用清化瘀毒方,对照组用复方鳖甲软肝片,疗程3个月。观察患者治疗前后主要症状与体征、肝功能、血清肝纤维化指标的改变。结果:治疗组患者治疗后,临床症状与体征较治疗前均明显改善或消失,差异有统计学意义(P<0.05);与对照组比较差异有统计学意义(P<0.05)。肝功能两组患者经治疗后均明显改善,与治疗前比较差异有统计学意义(P<0.01);组间比照,治疗组ALT、AST、GGT、TBiL、ALB改善较对照组为优,差异有统计学意义(P<0.01,P<0.05)。血清肝纤维化指标两组HA、PCⅢ治疗后改善明显,与治疗前比较差异有统计学意义(P<0.05,P<0.01);CⅣ、LN治疗组治疗后改善明显,与治疗前比较差异有统计学意义(P<0.01);对照组治疗后改善不明显;组间比较差异有统计学意义(P<0.05,P<0.01)。两组总疗效比较,治疗组总有效率93.3%;对照组总有效率66.7%。两组疗效经Ridit分析,u=2.51,P<0.05,差异有统计学意义。说明清化瘀毒方治疗效果优于对照组。结论:清化瘀毒方治疗酒精性肝硬化,不但可以改善患者临床症状与体征,恢复肝功能,并能有效抗肝纤维化,促进肝组织修复,对酒精性肝硬化有较好疗效,总有效率达93.3%。Objective： To investigate the clinical therapeutic effect of Qinghua Yudu Formula on alcoholic hepatocirrhosis of dampness-heat toxic stasis syndrome. Methods： 60 cases accorded with diagnostic criteria of alcoholic hepatocirrhosis and TCM syndrome of dampness-heat toxic stasis were divided into treatment group and control group randomly. The treatment group was treated with Qinghua Yudu Formula while the control group was treated with Fufang Biejia Ruangan Tablet. The course of treatment was three months. And main symptoms and signs, hepatic function and serum index of liver fibrosis before and after treatment were detected. Results： After treatment, clinical symptoms and signs in treatment group got obviously improved or disappeared compared with before treatment, there were significant difference（P〈0.05）. And there has significant difference compared with control group（P〈0.05）. The hepatic function improved of the two groups after treatment compared with before treatment, and there were significant difference（P〈0.01）. While the improvement of ALT, AST, GGT and TBIL in treatment group were significantly superior to those in control group（P〈0.01, P〈0.05）.The improvement of serum hepatic fibrosis index HA,PCIII of both groups made significant difference compared with that before treatment（P〈0.05, P〈0.01）. While the improvement of serum hepatic fibrosis index CIV,LN of treatment group made significant difference compared with that before treatment （P〈0.01）. There was also significant difference between the two groups on improvement of CIV and LN（P〈0.05, P〈0.01）. Total effective rate of treatment group was 93.3%, and control group was 66.7%. Analyzed by Ridit, P〈0.05, u=2.51, the difference between the two group was significant, indicating that the therapeutic effect of Qinghua Yudu Formula was better than control group.
