帕瑞昔布钠对老年患者脊柱外科术后肾功能的影响

摘　　要：目的:观察静脉注射帕瑞昔布钠(parecoxi-sodium)对老年患者脊柱外科术后肾功能的影响。方法:选择脊柱外科手术的老年患者(>65岁)80例,美国麻醉医师协会(ASA)Ⅰ～Ⅱ级,以随机抽样法均分为2组(每组40例):帕瑞昔布钠组(观察组)术后静脉注射帕瑞昔布钠40 mg,bid;对照组静脉注射等量0.9%氯化钠注射液,1日1次,共3 d。采集术后血、尿,监测肾脏功能的改变。结果:术后1、6 h,观察组与术前比较肌酐清除率降低(P<0.05),差异有统计学意义;对照组与术前比较则无明显变化(P>0.05);组间比较,观察组肌酐清除率低于对照组(P<0.05),差异有统计学意义。治疗期内,2组患者肾脏对钠、钾离子的分泌与尿白蛋白、β2微球蛋白指标均有短期升高,但组间差异无统计学意义(P>0.05)。结论:老年患者脊柱外科术后静脉注射帕瑞昔布钠对肾小球和肾小管的功能仅有短暂影响。OBJECTIVE ： To evaluate the effect of parecoxib sodium （ intravenous injection） on the renal function of elderly patients after undergoing spinal surgery. METHODS ： A total of 80 elderly patients aged above 65 years with ASA （American Societies of Anaesthesiologists ） I -1I grade undergoing spinal surgery were randomly assigned to either trial group or control group of 40 each. The trial group received intravenous parccoxib sodium （40 mg） b. i. d after undergoing spinal surgery and the control group received 0. 9% sodium chloride injection at the same amount by intravenous injection q. d for 3 days. The blood sample and urine sample were collected and the renal function was monitored. RESULTS： At 1 and 6 hours after surgery, creatinine clearance rate decreased in the trial group as compared with before surgery （P 〈 0. 05 ）, whereas the creatinine clearance rate decrease was not significant in the control group （ P 〉 0. 05 ） ; the creatinine clearance rate in parecoxib sodium group was lower than in the control group, and the differences between the two groups were statistically significant （ P 〈 0.05 ）. During treatment, urinary excretion of sodium, potassium, urine albumin and β2-microglobulin in both groups increased for a while, yet no statistical differences were noted between two groups （ P 〉 0.05 ）. CONCLUSION ： Intravenous injection of parecoxib sodium had transient impact on glomerular and tubular functions in elderly patients after undergoing spinal surgery.
