体外受精-胚胎移植治疗多囊卵巢综合征不孕患者临床效果

摘　　要：目的:分析体外受精-胚胎移植(IVF-ET)或卵胞浆内单精子注射(ICSI)对多囊卵巢综合征(PCOS)不孕妇女的助孕效果及PCOS不孕患者进行IVF-ET治疗过程中的特点。方法:回顾性分析2003年10月至2011年2月在郑州大学第二附属医院生殖医学中心接受IVF-ET/ICSI治疗的147名PCOS不孕患者157个周期[P-COS组IVF-ET术前均经过3～6个月的口服避孕药和(或)二甲双胍预治疗]和865例输卵管因素不孕患者925个新鲜周期(非PCOS组)的临床资料。结果:PCOS组促性腺激素(Gn)用量少于非PCOS组,PCOS组的取卵数、冷冻胚胎数、优质胚胎数均高于非PCOS组(P<0.05);PCOS患者1个辅助生育技术周期的药费[(8012±1038)元]少于非PCOS组[(10088±1207)元](t=19.653,P<0.001)。2组Gn使用时间、移植胚胎数、受精率、卵裂率、OHSS率等差异无统计学意义(P>0.05)。结论:PCOS患者术前经合理的内分泌调整会改善IVF-ET治疗结局。Aim：To study the effects of in vitro fertilization and embryo transfer（IVF-ET）or intracytoplasma sperm injection （ICSI） on infertility patients with polycystic ovary syndrome（PCOS） and the characteristic of PCOS in the IVF-ET course.Methods：To analyze the IVF-ET/ICSI informations of 147 patients with PCOS （157 cycles） and 865 cases （925 cycles） without PCOS （control） between Oct.2003 to Feb 2011. All patients with PCOS took oral conception（OC） or/and mateformine for 3 to 6 months before IVF-ET/ICSI.Results： In PCOS group,the number of gonadotrophin（Gn） used during the treatment was significantly less than that in the control group.In PCOS group, the number of oocyte retrieved and the cryopreserved embryos and total good embryos were all more than those in control（P0.05）.One assisted reproductive technology（ART） medicine cost in PCOS group was less than that in control（t=19.653,P0.001）.There were no differences in Gn using time and number of transfered embryos,pregnancy rate,multiple pregnancy rate and ovarian hyperstimulation syndrome rate between the two groups（P0.05）.Conclusion： The IVF-ET outcome in patients with PCOS would be improved after taking OC to modulate the endocrine situation before IVF-ET.
