373例乙肝肝硬化失代偿期患者证候与客观指标的相关性研究

摘　　要：目的:通过对乙肝肝硬化失代偿期患者进行回顾性研究,探索乙肝肝硬化失代偿期的证候特征与客观指标的相关性,为探索乙肝肝硬化失偿期的证候规律提供有力的证据。方法:运用回顾性研究,对373例2002年10月—2010年4月在广东省中医院肝病科、消化科的乙肝肝硬化失代偿期患者的资料,分析患者的中医证型与肝功能、HBV DNA定量、血常规等常见客观指标的相关性。结果:湿热蕴结型中ALT、AST、TBIL值明显高于其他各组,ALB值肝肾阴虚型明显低于肝郁脾虚证型,而GLB值肝肾阴虚型明显高于肝郁脾虚证型,HBV-DNA、AFP、血脂血糖、凝血、血常规进行了分析未发现之间的明显规律性。结论:肝功能是判断疾病变化发展的重要指标之一,对于协助中医辨证判断疾病的阶段及预后评估可起到一定的作用。Objective ：The hepatitis B patients with decompensated liver cirrhosis were studied retrospectively to explore the relevance of deeompensated liver cirrhosis syndrome characteristics and objective indicators to provide strong evidence for exploring the decompensation of cirrhosis syndromes. Metbo＆：A retrospective study of 373 cases in October 2002 to 2010 in April at the Liver Branch and Department of Gastroenterology in Guangdong Provincial Hospital of Traditional Chinese Medicine was made. The information of hepatitis B patients with decompensated cirrhosis was collected. The relevance of TCM syndrome and liv- er function, HBV DNA quantification, blood and other common objective indicators were analyzed. Results：ALT, AST,TBI1, in heat-dampness accumulation syndrome were significantly higher than that in other groups. ALB values ill kidney deficiency syn- drome were significantly decreased than that in liver depression and spleen deficiency syndrome, and GLB in kidney deficiency syndrome was significantly higher than the value of liver depression and spleen deficiency syndrome. Analysis on HBV-DNA, AFP, blood glucose, blood clotting and blood RT did not find the obvious regularity. Conclusion：The change in liver tunction is to determine the development of the disease, which is an important indicator of TCM to determine the stage of the disease and to play a role in the prognosis.
