中药泡洗结合针灸推拿和康复训练治疗脑梗死后肩手综合征的临床研究

摘　　要：目的:观察中药泡洗结合针灸推拿和康复训练治疗脑梗死后肩手综合征的临床疗效。方法:将122例脑梗死后肩手综合征的患者随机分为A、B两个队列,A队列(治疗组)62例和B队列(对照组)60例。A队列(治疗组)为中药泡洗+按摩+针灸+康复训练的综合康复训练方案,B队列(对照组)为单纯康复训练。分别于治疗前后进行简式Fugl-Meyer法进行上肢功能评分、Ashworth痉挛评分、疼痛数字评价量表(numerical rating scale,NRS)评分以及测量肩关节活动度范围。结果:两组治疗后与治疗前比较,均能不同程度地缓解疼痛,改善上肢运动功能(P<0.05或P<0.01),以及改善上肢痉挛和扩大肩关节活动范围(P<0.05或P<0.01),且治疗组的改善作用优于对照组(P<0.05或P<0.01)。结论:中药泡洗结合针灸推拿和康复训练可提高脑梗死后肩手综合征的治疗效果。Objective ：To observe the therapeutic effect of Chinese drugs bubble wash combined with acupuncture and massage and rehabilitation therapy for post-stroke shoulder-hand syndrome（SHS）. Methods ：122 post-stroke SItS patients were randomized into the treatment group （n = 62 ）and the Control group （n = 60 ）. The treatment group received Chinese drugs bubble wash com- bined with acupuncture and massage and rehabilitation therapy, and the control group received rehabilitation therapy alone. The pain score was evaluated with Numerical rating scale（NRS） ,the score of upper-limb function was evaluated with simplified Fugl- Meyer Assay,and the movement of shoulder joint was measured with rating scales. Resuhs：The pain was relieved,upper-limb func- tion was improved, and the movement of shoulder joint was increased to some extent after treatment in both groups （ P 〈 0. 05orP 〈 0. 01）,and the improvement of the above indexes in the treatment group exceeded that in the control group （P 〈 0. 05orP 〈 0. 01 ）. Conclusion：Chinese drugs bubble wash combined with acupuncture and massage and rehabilitation therapy has certain effi- ciency on post-stroke shoulder-hand syndrome.
