摘 要:目的观察原发性高血压(EH)和其合并T2DM患者血压节律及心脏结构功能的特点。方法随机选择EH组44例,EH合并T2DM(EH&T2DM)组50例,采集临床指标进行统计分析。结果 EH&T2DM组舒张末期左室内径(LV)和左室重量(LVM)较EH组升高,夜间收缩压下降率(SB-PF)、左室舒张早期与晚期二尖瓣最大血流速比值(E/A)和左室射血分数(LVEF)较EH组降低。EH&T2DM组血压非杓型变化发生率较EH组升高。非杓型变化的患者较杓型变化患者均出现LV增大,LVM增加。所有患者SBPF与舒张末期室间隔厚度(IVS)、LVM、LV呈负相关。结论EH&T2DM时容易出现血压昼夜节律异常,对心脏结构和功能的影响更为明显。血压昼夜节律异常可能是高血压患者心脏结构和功能变化的主要原因之一。Objective To investigate the characteristics of rhythm of blood pressure and cardiac structure-function of essential hypertension (EH) patients and EH patients complicated with T2DM. Methods The subjects were selected randomly. Forty-four EH patients were in the EH group and fifty EH patients complicated with T2DM in the EH^T2DM group. Their clinical data were collected for comparison, which was analyzed with SPSS 16. Results Compared with the EH group, the patients in the EH&T2DM group had an increase of LV and LVM, a decrease of SBPF, E/A and LVEF, and a higher radio of non-dipper rhythm of blood pressure. There existed an increase of LV and LVM in the patients with non-dipper rhythm of blood pressure than in the patients with dipper rhythm of blood pressure. In all patients, the SBPF was negatively correlated with IVS, LVM, and LV. Conclusion The EH patients with glycometabolism disorders usually have the abnormal rhythm of blood pressure that affects the cardiac structure-function more obviously. The abnormal rhythm of blood pressure may be one of main reasons of EH patients' cardiac structural and functional changes.
