摘  要:目的比较异丙酚或依托咪脂复合芬太尼麻醉在小儿孤独症干细胞移植术中的应用,评估其临床有效性和安全性。方法选择择期行腰椎穿刺干细胞移植术治疗的孤独症患儿120例,随机将其分为异丙酚-芬太尼(PF)组和依托咪脂-芬太尼(EF)组,每组60例,两组均给予芬太尼1.0μg/kg,PF组给予异丙酚2.0 mg/kg,EF组给予依托咪脂0.5 mg/kg。观察记录平均动脉压(MAP)、心率(HR)、脉搏血氧饱和度(SPO2)的变化,术后睁眼时间(停药至呼之能睁眼),完全苏醒时间(哭闹)和术后并发症如躁动、恶心、呕吐等。结果两组睁眼时间和完全苏醒时间差异无统计学意义(P>0.05);PF组术中呼吸抑制和术后躁动发生率明显高于EF组(P<0.05),EF组血压和心率波动也较PF组轻(P<0.05)。结论依托咪脂复合异丙酚麻醉是孤独症患儿行腰椎穿刺干细胞治疗的良好选择之一。Objective To compare the clinical efficacy and safety of Propofol-Fentanyl with Etomidate-Fentanyl anesthesia for autistic children performing stem cell transplantation. Methods 120 autistic children scheduled for stem cell transplan- tation, were randomly divided into Propofol-Fentanyl (PF) group and Etomidatep-Fentanyl (EF) group, 60 cases in each group. The two groups were all given Fentanyl 1.0 μg/kg, PF group were injected Propofol 2.0 mg/kg, EF group were slowly injected Etomidate 0.5 mg/kg. The mean arterial pressure (MAP), heart rate (HR), changes in pulse oximetry (SPO2), time to open the eye when called, fully awake time (crying) and postoperative complications such as agitation, nausea, and vomiting were recorded. Results Time to open in two groups was no difference (P 〉 0.05), incidence of respiratory depression and postoperative agitation in PF group of patients was significantly higher than in EF group, fluctuation of blood pressure and heart rate in EF group was more lightly than that in PF group. Conclusion Etomidate-Fentanyl anesthesia is a good choice for autistic children performing stem cell transplantation.
