斜卧位经皮肾镜超声气压弹道碎石在治疗复杂肾输尿管上段结石的临床应用及其对患者性功能等方面的影响研究

摘　　要：目的:探讨斜卧位经皮肾镜超声气压弹道碎石在治疗复杂肾输尿管上段结石的临床效果及其对患者性功能等生活质量方面的影响。方法:将2009年1月～2011年6月于本院进行经皮肾镜超声气压弹道碎石术治疗的62例复杂肾输尿管上段结石患者随机分为对照组(俯卧位组)和观察组(斜卧位组)各31例,将两组患者的一期结石清除率、手术时间、并发症发生率、造瘘管留置时间、住院时间、满意率及术前、术后1个月患者性功能等生活质量方面进行评估及比较。结果:观察组的一期结石清除率、手术时间、并发症发生率、造瘘管留置时间、住院时间、满意率、生存质量及性功能评分均优于对照组,P<0.05,均有显著性差异。结论:斜卧位经皮肾镜超声气压弹道碎石在治疗复杂肾输尿管上段结石的临床效果要好于俯卧位手术。Objectives： To study the clinical effect of ultrasonic and pneumatic lithotripsy surgery by pereutaneous nephro-lithotomy at oblique position in the treatment of complex renal and ureteral calculi and to evaluate the effects of surgery on the sex-ual life quality of the patients. Methods： A total of 62 patients with complex renal and ureteral calculi were randomly divided into the control group（ prone position group）and the observation group（ oblique position group）who were treated with ultrasonic and pneumatic lithotfipsy surgery by percutaneous nephrolithotomy from January 2009 to June 2011. Each group included 31 cases. The two groups were compared in terms of the Stage I stone clearance rate, operation time, incidence of complications, indwelling time of fistula, hospitalization time, satisfaction rate and the sexual life quality before and one month after the surgery. Results： The observation group showed significant better effects than the control in the Stage I stone clearance rate, operation time, inci- dence of complications,indwelling time of fistula, hospitalization time, satisfaction rate, and sexual function score （ P 〈 0. 05 ）. Conclusion ：The clinical effect of ultrasonic and pneumatic lithotfipsy surgery by percutaneous nephrolithotomy at oblique position is better than the effect of prone positionin surgery in the treatment of complex renal and ureteral calculi.
