摘 要:目的采用不同方法对2006~2008年北京地区流脑病例进行核酸检测并对结果进行比较分析。方法收集北京地区流脑病例的抗凝血标本和脑脊液标本,利用普通多重PCR和多重实时荧光PCR进行流脑病例病原学检测。结果 (1)2006~2008年,北京市疾病预防控制中心免疫预防所实验室共收集到79例流脑病例标本。脑膜炎奈瑟氏菌、肺炎链球菌和B型流感嗜血杆菌三者总检出率为31.65%(25/79),其中脑膜炎奈瑟氏菌检出率为29.11%(23/79),占总阳性率的92%(23/25);肺炎链球菌检出率为2.53%(2/79),占总阳性率的8%(2/25),B型流感嗜血杆菌未检出。脑膜炎奈瑟氏菌血清群分布A群、C群和未定型所占比例分别为:65.22%、26.08%、8.70%。(2)流脑多重实时荧光PCR检测方法的灵敏性明显优于流脑多重普通PCR;(3)抗凝血标本脑膜炎奈瑟氏菌检出率明显低于脑脊液标本。结论北京地区引起流脑病例的主要病原菌血清型为A和C群。仅检测流脑病例血液标本可能会造成漏检。OBJECTIVE To compare and analysis the test results of the suspected eases of meningococcal meningitis in Beijing from 2006 to 2008. METHODS Clinical specimens, including blood specimen and CSF, were collected from the meningoeoc cal meningitis suspected cases in Beijing. The specimens were detected for pathogens with multiplex conventional PCR and mul tiplex real-time PCR. RESULTS First, the total positive rate for meningococcal meningitis, Streptococcus pneumonia and haemophilus influenza type b was 31.65% (25n9). The positive rate for meningococcal meningitis alone is 29.11% (23n9), and its proportion is 92% (23/25) among the total positive rate; the positive rate for Streptococcus pneumonia alone is 2.53% (2/79), and its proportion was 8% (2/25) ; No haemophilus influenza type B was detected. The proportion of meningococcal meningitis was 65.22% for type A, 26.08% for type C, 8.70% for type non A non C, respectively. Second, multiplex real time PCR assay was more sensitive than multiplex conventional PCR assay. Finally, the positive rate for meningococcal meningi tis detection of CSF specimens was higher than that of blood specimens, though no statistic difference was observed because of too small number of sample involved. CONCLUSION According to the results above, we found that the serogroup A and C were the major pathogens for meningococcal meningitis in Beijing.
