抗α-胞衬蛋白抗体在干燥综合征诊断和鉴别诊断中的作用

摘　　要：目的探讨抗α-胞衬蛋白(Fodrin)抗体在干燥综合征诊断和鉴别诊断中的作用。方法应用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)定量检测72例原发性干燥综合征(pSS),32例继发性干燥综合征(sSS),78例系统性红斑狼疮(SLE),120例类风湿关节炎(RA)患者和60例正常健康体检者血清中抗α-Fodrin IgA、IgG抗体。结果 pSS中的抗α-fodrin IgA和IgG抗体阳性率高于sSS(P﹤0.05);pSS和sSS患者中抗α-Fodrin IgA、IgG抗体的阳性率均显著高于SLE和RA患者(P﹤0.05)。抗α-Fodrin IgA抗体诊断干燥综合征的灵敏度为55.8%,特异性为97.5%;而抗α-Fodrin IgG抗体诊断干燥综合征的灵敏度为44.2%,特异性为99.0%。结论抗α-Fodrin抗体特异性很好,对干燥综合征的诊断和鉴别诊断具有重要的临床意义,尤其对于ANA、抗SS-A抗体、抗SS-B抗体均阴性的患者,常规筛查抗α-Fodrin抗体,有利于干燥综合征的早期诊断。OBJECTIVE To investigate the significance of anti-α-Fodrin antibody in the diagnosis and differential diagnosis of SjOgren＇s syndrome. METHODS Serum antiα-Fodrin IgA and IgG antibodies of patients w/th pr/mary Sjogren＇s syndrome （pSS, n = 72）, secondary Sjogren＇s syndrome （sSS, n = 32）, systemic lupus erythematosus （SLE, n = 78）, rheumatoid arthritis （RA, n = 120） and normal controls （n = 60） were detected by enzyme-linked immunosorbent assay （ELISA）. RE- SULTS The positive rates of anti-α-Fodrin IgA and IgG antibodies in primary and secondary SS were significantly higher than those in SLE, RA and normal controls （P 〈 0.05）. The sensitivities of anti-α-Fodrin IgA and IgG antibodies were 55.8% and 44.2%, respectively. The specificities of anti-a-Fodrin IgA and IgG antibodies were 97.5% and 99.0%, respoctively. CON CLUSION As a new diagnostic marker for patients with SS, anti-α-Fodrin antibody had high specificity. It is helpful for the diagnosis of SS especially for patients with negative ANA. anti-SSA and anti-SSB antibodies.
