某院2008～2011年下呼吸道感染患者铜绿假单胞菌检测及耐药性变迁

摘　　要：目的了解某院在2008年1月～2011年12月连续4年下呼吸道感染患者中铜绿假单胞菌检测结果及耐药性变迁趋势。方法采用法国梅里埃公司生产的VITEK-AMS全自动微生物分析仪进行细菌鉴定,纸片扩散片(K-B)法进行药物敏感试验。结果共分离出铜绿假单胞菌2997株,其中2008年431株(14.38%),2009年600株(20.02%),2010年1294株(43.18%),2011年672株(22.42%);铜绿假单胞菌对氨苄西林、头孢唑林、头孢西丁、头孢呋辛钠、头孢呋辛酯耐药性逐年上升,并且已经处于完全耐药,对阿米卡星、头孢吡肟、头孢哌酮/舒巴坦、头孢噻肟、庆大霉素、亚胺培南、左氧氟沙星、美罗培南、哌拉西林/他唑巴坦的耐药性在4年之间有升有降,但是整体分析耐药性处于上升状态,头孢他啶耐药性有下降趋势。结论下呼吸道感染患者入院后应早期进行细菌培养确定感染病原体,依据药物敏感试验结果选择抗菌药物,在药物敏感试验结果尚未出来前尽可能选用喹诺酮类及氨基糖甙类抗生素为宜。OBJECTIVE To understand pseudomonas aeruginosa test results and resistance change trend four consecutive pa tients t：with respiratory tract infection from oar hospital from January 2008 to December 2011. METHODS VITEK merry ella AMS automatic microbial bacteria analyzer identification was used, scraps of paper spread sheet （K-B） method was used for the drug susceptibility testing. RESULTS There were separate pseudomonas aeruginosa 2997 plants, including 431 to strains （14.38%） in 2008, 600 strains （20.02%） in 2009, 1294 strains （43.18%） in 2010, 672 strains （22.42%） in 2011; Pseudomonas aeruginosa ampicillin, cefazolin, cefazolin Xiding, cefuroxime sodium, cefuroxime ester resistance were rising year by year, and had been in a fully resistance to ahmed card to star, cefepime, cefoperazone/sulbactam, cefazol/n latter, gentamycin, imipenem, levofloxacin, beauty e.faecalis and piperacillirdsulbactarn in the four years, hut the overall drug resis tance was increased, cefotaxime resistance was declined. CONCLUSION The patients with respiratory tract infection after admission to determine the detaiLs should be early training infection pathogens, according to the medicine sensitive test results an timicrobial choice in drug sensitive experiment. Quinolone and amino sugar glucoside class antibiotic are suggested to be used in clinics.
