血浆hs-CRP、BNP、vWf对心源性脑梗死复发的预测作用

摘　　要：目的探讨血浆超敏C-反应蛋白（hs—CRP）、血浆脑钠素（BNP）、血浆血管性血友病因子（vWf）对心源性脑梗死复发的预测作用。方法通过对急性心源性脑梗死（CE）50例检测血浆hs—CRP、BNP、vWf水平．进行1年以上的随访，分别按血浆hs—CRP水平的中位数分为低hs—CRP组（hs—CRP≤355mg／L）和高hs—CRP组（hs—CRP〉3．55mg／L），按血浆BNP水平的中位数分为低BNP组（BNP≤322．80pg／d1）及高BNP组（BNP〉322．80pg／d1），按血浆vWf水平的中位数分为低vWf组（vWf≤3352ng／ml）及高vWf组（vWf〉3352ng／ml）。分别进行两两比较，观察急性期hs—CRP、BNP、vWf水平对CE复发的预测作用。结果50例CE患者排除他因死亡3例，其余47例中复发11例，复发率23．4％。低hs—CRP组31例。复发8例（25．8％），高hs—CRP组16例，复发3例，（188％），比较两组间基础情况均无统计学意义（P〉0．05），高、低hs—CRP组的复发生存曲线比较无统计学意义（P〉0．05）。低BNP组34例，复发4例（11．8％）。高BNP组13例，复发7例（53．8％），两组间基础情况比较无统计学意义（P〉0．05），高、低BNP组的复发生存曲线比较有统计学意义，高BNP组随时间复发率明显高于低BNP组（P〈0．05）。低vWf组32例，复发8例（25％），高BNP组15例，复发3例（20％），两组间基础情况比较无统计学意义（P〉0．05），高、低vWf组的复发生存曲线比较无统计学意义（P〉0．05）。结论脑梗死急性期血浆BNP的水平可能对CE的复发有一定预测意义，而血浆hs—CRP、vWf水平无明显预测CE复发的作用。Objective TO investigate the plasma levels of hs-CRP, BNP and vWf in predicting the recurrence of cardioembolic stroke. Methods Plasma hs-CRP, BNP and vWf levels were measured in 50 patients with acute cardioembolic stroke （CE）, the patients were followed up for more than 1 year. Results Among 50 patients 3 died due to other reasons, in the remaining 47 patients, CE recurred in 11 cases with an overall recurrence rate of 23.4%. The recurrence rate in low hs-CRP group （hs-CRP ≤3.55mg/l） was 25.8% （8/31）, while that in high hs-CRP group （hs-CRP〉3.55mg/I） was 18.8% （3/16） （P 〉0.05）. The recurrence rate in low BNP group （BNP~〈322.80pg/dl） was 11.8% （4/34）, while that in high BNP group（BNP〉322.80pg/dl） was 53.8%（7/13）（P〈0.05）. The recurrent rate in low vWf group （vWf≤33.52ng/ml） was 25.0% （8/32）, while that in high vWf group （vWf〉33.52ng/ml） was 20.0%（3/15）（P 〉0.05）. Conclusion The plasma BNP levels may be of value in prediction of recurrence n patients with acute cardioembolic stroke.
