IGF-Ⅰ、IGF-Ⅱ、IGFBP-1与妊娠期高血压疾病的相关性研究

摘　　要：目的研究妊娠期高血压疾病(hypertensive disorder complicating pregnancy,HDCP)与胰岛素样生长因子-Ⅰ(insulin-like growth factor-Ⅰ,IGF-Ⅰ)、胰岛素样生长因子-Ⅱ(insulin-like growth factor-Ⅱ,IGF-Ⅱ)、胰岛素样生长因子结合蛋白-1(insulin-like growth factor binding protein-1,IGFBP-1)的相关性,为HDCP的病因学研究提供理论依据。方法采用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)检测脐血和羊水中IGF-Ⅰ、IGF-Ⅱ、IGFBP-1水平。正常孕妇16例(对照组),HDCP孕妇48例(研究组),其中妊娠期高血压17例,轻度子痫前期16例,重度子痫前期15例。结果研究组较对照组IGF-Ⅰ、IGF-Ⅱ水平均显著下降(P<0.05)、IGFBP-1水平显著升高(P<0.05);研究组内IGF-Ⅰ和IGF-Ⅱ水平随病情程度加重而降低、IGFBP-1水平随病情程度加重而升高,不同病情程度之间比较差异有统计学意义(P<0.05)。研究组IGF-Ⅰ和IGF-Ⅱ呈正相关,IGF-Ⅰ、IGF-Ⅱ均与IGFBP-1呈负相关(P<0.05)。结论IGF-Ⅰ、IGF-Ⅱ、IGFBP-1与HDCP的发生和病情严重程度密切相关。Objective To explore the correlation between insulin- like growth factor-Ⅰ （IGF -Ⅰ ）, insulin - like growth factor-Ⅱ （ IGF - Ⅱ）, insulin - like growth factor binding protein - 1 （ IGFBP - 1 ） and hypertensive disorder complicating pregnancy （HDCP）. Methods The levels of IGF - Ⅰ, IGF - Ⅱ and IGFBP - 1 in umbilical cord blood and amniotic fluid were detected by enzyme linked immunosorbent assay （ELISA）. There were 16 normal pregnant women in control group, while 48 pregnant women in experimental group, including 17 with hypertension, 16 with mild preeclampsia, 15 with severe preeclampsia. Results In experimental group, the levels of IGF - Ⅰ and II were significantly lower than those in control group （ P 〈 0. 05 ） , and the level of IGFBP - 1 was statistically andsignificantly higher （ P 〈 0. 05 ）. The levels of IGF - Ⅰ and IGF - Ⅱ in experimental group decreased as the condition of patients was getting worse and worse, while IGFBP - 1 was in opposite. There was a significant and positive correlation between the levels of IGF - Ⅰ and IGF - Ⅱ, while negative correlations existed between IGF - I and IGFBP - 1 in the experimental group, as well as between IGF - II and IGFBP - 1 （ P 〈 0. 05 ）. Conclusion The levels of IGF - Ⅰ, IGF - Ⅱ, IGFBP - 1 were closely related with the occurrence of HDCP and its severity.
