摘 要:目的观察调补肺肾方法(补肺健脾、补肺益肾、益气滋肾)对慢性阻塞性肺疾病(chronicobstructive pulmonary disease,COPD)大鼠肺组织胶原和基质金属蛋白酶(matrix metalloproteinases,MMPs)表达的影响及远后效应。方法将大鼠随机分为正常对照组、模型组、补肺健脾组[4.84 g/(kg.d)]、补肺益肾组[4.44 g/(kg.d)]、益气滋肾组[4.84 g/(kg.d)]和氨茶碱组[2.3 mg/(kg.d)],采用香烟熏吸联合细菌感染法制造COPD稳定期模型。于第9周起分别给予补肺健脾方、补肺益肾方、益气滋肾方和氨茶碱,给药12周,于第20、32周分批取材,观察肺组织病理、MMP-9、MMP-2、金属蛋白酶组织抵制剂(TIMP)-1和Ⅰ、Ⅲ、Ⅳ型胶原等表达。结果模型组肺组织病理损害明显。第20周时,补肺健脾、补肺益肾、益气滋肾中药可明显减轻肺组织病理损伤,改善支气管壁增厚、支气管狭窄、肺泡破裂和融合、肺小动脉壁增厚(P<0.05或P<0.01),降低肺组织MMP-9、MMP-2、TIMP-1和Ⅰ、Ⅲ、Ⅳ型胶原等表达(P<0.05或P<0.01),在32周仍显著改变(P<0.05或P<0.01),且较氨茶碱组显著(P<0.05或P<0.01)。结论补肺健脾、补肺益肾、益气滋肾法可明显改善COPD肺组织损害,降低肺组织胶原和基质金属蛋白酶表达,且远后效应良好。Objectives To observe the influence and long-term effect of therapies of regulating and supplementing lung and kidney (tonifying lung and spleen, tonifying lung and replenishing kidney, and replenishing qi and enriching kidney) on collagen and matrix metalloproteinase (MMP) in lung tissue in rats with chronic obstructive pulmonary disease (COPD). Methods All rats were randomly divided into normal group, model group, tonifying lung and spleen group (treatment group 1 ), tonifying lung and replenishing kidney group (treatment group 2 ) , replenishing qi and enriching kidney group (treatment group 3 ) and aminophyline group. The model of stable COPD was established by using the method of cigarette inhalation combining bacterial infection. From the 9th week all groups were respectively given corresponding medicinals until to the 20th week. The samples were collected on the 20^th and 32nd week respectively. The pathological changes of lung tissue and expressions of MMP-9, MMP-2, TIMP-I, collagen I, collagen III and collagen IV were observed. Results In model group there were obvious pathological injuries was observed in lung tissue, and on the 20th week this injuries, including thickening of bronchial wall, bronchial stenosis, alveolar disruption and fusion, and thickening of pulmonary arteriolar wall, were relieved significantly in treatment group 1, treatment group 2 and treatment group 3 (P 〈 O. 05 or P 〈 0. O1 ). The expressions of MMP-9, MMP-2, TIMP-1, collagen I, collagen [[[ a^ld collagen IV were reduced (P 〈0.05 or P 〈0.01 ), which still were significant on the 32nd week (P 〈 0.05 or P 〈0.01 ) and were more significant than those in aminophyline group (P 〈0.05 or P 〈0.01 ). Conclusion The therapies of tonifying lung and spleen, tonifying lung and replenishing kidney, and replenishing qi and enriching kidney can obviously relieve the impairments of lung tissue induced by COPD, reduce the expressions of collagen and matrix metalloproteinase with a good long
