髓芯减压植骨内支撑术结合中药治疗早期股骨头坏死的临床观察

摘　　要：目的探讨髓芯减压植骨内支撑术结合中药对早期股骨头坏死的临床疗效。方法将24例(34髋)Ⅰ、Ⅱ期股骨头坏死患者随机分成治疗组和对照组各12例,2组均采用髓芯减压植骨内支撑术治疗,治疗组同时结合中药治疗,疗程6个月。观察两组患者治疗前后的Harris积分、影像评分、疗效总积分等指标。结果治疗组总有效率为94.4%,对照组总有效率87.5%,治疗组明显高于对照组(P﹤0.05)。治疗后治疗组Harris评分、影像学评分、疗效总评分较治疗前均明显增加(P﹤0.01),与对照组治疗后比较,差异有统计学意义(P﹤0.05)。结论髓芯减压植骨内支撑术结合中药治疗早期股骨头坏死较单纯髓芯减压植骨内支撑术治疗能较好地减轻甚至消除患髋疼痛、改善痛髋活动度和下肢生理功能,促进了骨组织再生及修复,加快了患者的康复,值得临床推广。Objective To explore the clinical effects of core decompression port bone grafting combined with Traditional Chinese Medicine on early and internal supnecrosis of the femoral head. Methods 24 patients with early necrosis of the femoral head were randomly divided into treatment group and control group, with 12 cases in each group. 2 groups were both treated with core decompression and internal support bone grafting, and treatment group was treated with TCM therapy at the same time. The treatment course was 6 months. Harris Score, image score and total efficiency score were observed before and after treatment. Results After treatment, total effective rate was 94.4% in treatment group which was much higher than 87.5% in control group （P〈 0.05）. After treatment, Harris Score, image score and total efficiency score were increased in treatment group （P〈 0.01）; compared with control group, the differences had statistic significance （P〈 0.05 ）. bone Conclusion Compared with the method of core decompression and internal support grafting, the method of core decompression and internal support bone grafting combined with TCM has better effects in treating early necrosis of the femoral head. The integrated method can release the pain of pelvis, improve the flexibility of pelvis and physical functions of lower limb, promote repairing and regeneration of bone tissue, and therefore promote the recovering. It is worth being applied in clinic.
