负压封闭引流在胰腺十二指肠损伤中的应用

摘　　要：目的探讨负压封闭引流技术应用在胰腺十二指肠损伤治疗中的效果。方法回顾性分析近9年23例胰腺十二指肠损伤患者应用负压封闭引流治疗的临床资料。结果 2例发生胰瘘,3例十二指肠瘘,经负压封闭引流治愈各1例,6例再次手术行瘘管空肠吻合治愈;无死亡,无腹膜后或腹腔感染等并发症发生。结论严重胰腺十二指肠损伤治疗成功的关键是早期诊断、及时和适当的手术,负压封闭引流是充分及高效的引流,能简化控制性手术,防止腹膜后或腹腔感染等并发症,有效地治疗胰腺或十二指肠瘘。【Objective】 To investigate the treatment effect with application of vacuum sealing drainage in pancreatic and duodenal injury.【Methods】 The clinical data of 23 cases in recent 9 years with pancreatic and duodenal injury treated by application of vacuum sealing drainage were retrospectively analyzed.【Results】 2 cases of pancreatic fistula,3 cases of duodenal fistula,each of the 1 cases were cured by vacuum sealing drainage.6 cases were cured by operation of fistula-jejunal anastomosis.No death,no retroperitoneal or intraperitoneal infection and other complications happened.【Conclusion】 Early diagnosis,timely and appropriate operation are the key of successful treatment in severe pancreatic and duodenal injury.Vacuum sealing drainage is adequate,efficient drainage.It can simplify the control surgery,prevent retroperitoneal or intraperitoneal complications such as infection,and is effective in the treatment of pancreatic or duodenal fistula.
