农村不同人群CES-D抑郁量表信度及效度评价

摘　　要：目的评价流调中心抑郁水平评定量表(CES-D)在农村自杀死亡和正常对照人群信息人测量的信度和效度。方法采用序贯抽样方法对山东省邹城、高唐、莒南县抽取的202例自杀死亡病例和271名正常对照人群的信息人进行问卷调查;采用Cronbach'sα系数、相关系数评价量表的内部一致性信度,采用结构效度、效标效度和区分效度来评价量表的效度。结果 CES-D量表在病例组和对照组信息人中的Cronbach'sα系数分别为0.917和0.849,各条目得分与删除此条目后量表得分的相关系数分别为0.255～0.815和0.168～0.684(均P<0.01),量表的内部一致性较好;探索性因子分析表明,20个条目提取出情感障碍和躯体症状、积极情绪和人际冲突3个因子,累积方差贡献率为52.539%;对随机抽取的69名对照组信息人进行效标效度分析,结果表明CES-D量表情感和躯体症状、积极情绪因子和抑郁总分与Beck绝望量表(BHS)绝望得分均呈正相关关系(P<0.05),量表具有较好的效标效度;473名调查对象中抑郁者59例,非抑郁者414人,抑郁组情感障碍和躯体症状、积极情绪、人际冲突和抑郁总分分别为(17.44±5.86)、(9.00±4.51)、(0.88±1.68)和(27.32±9.05)分,均高于非抑郁组的(1.73±2.80)、(1.20±1.97)、(0.08±0.42)和(3.00±3.92)分,差异均有统计学意义(P<0.001),量表具有较好的区分效度。结论 CES-D量表具有较好的信度和效度,适用于农村不同人群抑郁的评价。Objective To assess the reliability and validity of Center for Epidemiological Studies Depression Scale（CES-D） among the informants of suicides and controls.Methods Totally 202 suicide informants and 271 control informants were consecutively sampled and investigated with the questionnaire.Cronbach＇s α and Pearson＇s correlation were used to assess the internal consistency reliability.Exploratory factor analysis,criterion validity and discriminant validity were used to test the validity of the scale.Results The Cronbach＇s α of CES-D were 0.917 and 0.849 in case and control group,respectively.The corrected item-total correlations in suicides and controls were 0.255-0.815 and 0.168-0.684,respectively（both P0.01）,suggesting the scale had good internal consistency reliability.Three factors were extracted by using exploratory factor analysis from 20 items（including affective and somatic symptoms,positive emotion,and interpersonal problems）,which could explain 52.539% of total variance.In addition,69 samples were randomly sampled from the control informants.There were positive correlations between affective and somatic symptoms,positive emotion,CES-D and Beck Hopelessness Scale（BHS）.The scale showed good criterion validity.There were 59 depressed subjects and 414 non-depressed subjects among the 473 informants.The scores of affective and somatic symptoms,positive emotion,interpersonal problems and CES-D were 17.44±5.86,9.00±4.51,0.88±1.68,and 27.32±9.05 in depression group,which were all higher than non-depression group（1.73±2.80,1.20±1.97,0.08±0.42,and 3.00±3.92）,with significant differences between depression and non-depression group（all P0.001）.The scale was proved having good discriminant validity.Conclusion CES-D has high reliability and validity and is applicable to assess depression in rural residents.
