摘　　要：基本医疗和公共卫生服务是保障和改善人们生命与健康权的基本条件。受承担法律责任的要求,受构建服务型政府的要求,受基本医疗和公共卫生服务性质的要求以及受保障健康权、维护人权的要求,政府应承担起提供基本医疗和公共卫生服务的责任。由于受历史原因、体制原因及其他因素的影响,我国地区、城乡基本医疗和公共卫生服务不均等的状况较为突出。医疗卫生服务不均等的主要原因包括地区、城乡经济发展不均衡;软约束的市场化改革;城乡二元结构的体制性约束。这就需要加强对中西部地区的财政转移支付;建立政府主导下的市场机制发挥作用的医疗卫生体制;逐步化解城乡二元结构。Basic medical and health services are of great importance to people~ lives and health. Due to under- going legal responsibility, constructing service - oriented government, characteristic of basic medical and health services, and protecting human right, it is necessary for the governments to take the responsibility of providing basic medical and health services. For the reason of history and system, the practical disparity of regional basic medical and health services and urban and rural basic medical and health services is larger and larger, which results from regional and urban and rural inequality of economic development, market reform of soft constraints, imperfect re- sponsibility of governments and dual structure in urban and rural areas. It is of great necessity to strengthen the fis- cal transfer for the central and western region, construct marketing medical and health system under the domination of governments, and solve urban and rural structure step by step.
