摘  要:目的:观察盐酸戊乙奎醚用于吸烟患者开胸手术对肺功能的影响。方法:60例吸烟患者择期在全麻下行胸部手术的患者,随机分为盐酸戊乙奎醚组(P组,n=30)和对照组(C组,n=30)。P组入麻醉诱导前静脉给予盐酸戊乙奎醚0.02mg.kg-1,C组则给予等量生理盐水。分别记录气管插管后切皮前(T1)、单肺通气后30min(T2)、单肺通气后60min(T3)、恢复双肺通气后15min(T4)的气道峰压(Ppeak)、气道平台压(Pplat)、肺顺应性(Compl)、气道压力(Paw)以和各时点的血气分析参数变化。结果:P组患者在T2、T3、T4时肺顺应性降低的幅度及气道峰压、气道平台压、气道阻力升高的幅度明显低于C组(P<0.05)。P组在T2、T3、T4时的SaO2明显高于C组(P<0.05)。结论:盐酸戊乙奎醚能够改善吸烟患者开胸手术中的呼吸力学、减轻肺损伤,从而达到肺保护作用。Objective. To investigate the effect of penehyclidine hydrochloride on pulmonary func- tion of smoking patients undergoing thoracic surgery. Methods. Sixty smoking patients scheduled for tho- racic surgery were randomly divided into penehyclidine hydrochloride group(group P) and control group (group C) with 30 patients in each group. The patients in group P received intravenous penehyclidine hydrochloride at the dosage of 0.02 mg/kg before induction and the patients in group C received equal vol- ume of physiological saline. Arterial blood samples were taken and Ppeak,Pplat,Compl,Paw were recorded at the time points of before operation(T1 ),30min(T2) .60min (T3) after one-lung ventilation, 15 min after double lung ventilation (T4). Results:The Cmpl .Ppeak.Pplat and Paw in group P was significantly lower than in group C at T2 .T3 .T4 (P〈0.05). The SaOz in group P was significantly higher than in group C at T2 .T3 .T4 (P〈0. 05). Conclusion: Penehyclidine hydrochloride can improve the pneumodynamics, reduce lung injury and protect pulmonary function in smoking patients undergoing thoracic surgery.
