摘 要:目的:通过观察冠心病患者调节性T细胞(Tregs)和树突状细胞(DCs)数量和程序性死亡受体-1(PD-1)及其配体(PD-L1)的表达变化,探讨其功能状态,为动脉粥样硬化及不稳定斑块形成的病因及免疫疗法提供思路。方法:入选冠心病患者39例,其中稳定型心绞痛(SA)23例、急性冠状动脉综合征(ACS)16例;20例非冠心病患者为对照组。采用流式细胞术测定各组外周血CD4+CD25hi CD127low PD-1+Treg数量和CD11c+PD-L1+DC数量;用ELISA法测定血清IL-2水平。结果:外周血Treg数量在ACS组、SA组和对照组间差异无统计学意义;ACS组Treg PD-1表达率和DC、PD-L1表达率与SA组和对照组相比明显降低,差异有统计学意义(P<0.05)。结论:PD-1/PD-L1表达下调提示Treg和DC免疫抑制功能缺陷,可能是促进动脉粥样硬化病变进展及不稳定斑块形成的因素。Objective:To investigate the quantity of Tregs/dendritic cells (DCs) and their corresponding PD-1/ PD-L1 expression, and reveal the different functional status of both Tregs and DCs in patients with stable athero- sclerosis or acute coronary syndrome. Method.The 39 patients with coronary heart disease (CHD) were enrolled, including 23 stable angina (SA) patients and 16 acute coronary syndrome (ACS) patients. Twenty non-CHD indi- viduals proved by coronary angiography were selected as controls. The quantities of CD4+ CD25+ CD1271~wPD-1+ T cells and CDllc+ PD-L1+ DCs were determined by flow cytometry. The serum levels of IL-2 were measured by ELISA. Result .. There was no significant difference of Treg quantity in CHD patients (including ACS and SA patients) and healthy controls. However, we found the expression of PD-1 on Tregs and PD-L1 on DCs in ACS patients were significantly down-regulated compared with SA and controls. Conclusion.. Regulatory T cells may exhibit immune suppressive function via PD-1 molecule. Impaired function of Treg and DC may promote the pro- gress of atherosclerosis and the vulnerability of plaque.
