摘 要:目的:应用DWI初步评估胎儿脑发育情况。方法:分析30例正常胎儿脑的DWI表现。30例孕妇,年龄23~40岁,平均28岁;孕龄22.6~36.9周,平均30.1周。行MRI及DWI检查,测量额叶、顶叶、颞叶、枕叶、小脑和丘脑的ADC值。结果:30例胎儿额叶、颞叶、枕叶、顶叶、小脑和丘脑的ADC值,随着胎龄的增加而降低,小脑的相关系数最高,其次是丘脑,然后依次是枕叶、颞叶、顶叶、额叶。随孕周增加ADC值下降最快的是小脑,其次是丘脑,然后依次是枕叶及颞叶、顶叶、额叶。顶叶的平均ADC值最高,然后依次是枕叶、额叶及颞叶、小脑、丘脑。结论:DWI可初步评估胎儿脑发育情况。随着胎龄增加,胎儿脑组织逐渐成熟。小脑与丘脑的成熟早于幕上脑叶。Purpose: To study the diffusion-weighted imaging (DWI) in evaluation of the developing normal fetal brain. Methods: Thirty pregnant women, aged from 23-40 years (average 28 years) and with gestation from 22.6-36.9 weeks (average 30.1 weeks) were studied with MRI and DWI. Apparent diffusion coefficient (ADC) values were obtained in the frontal, parietal, occipital and temporal lobe, the cerebellar hemisphere and the thalamus of them. Results: The ADC values of frontal, parietal, occipital and temporal lobe, cerebellar hemisphere and thalamus showed a linear negative correlation with gestational age. The correlation coefficient was the highest in the cerebellum, followed by the thalamus, the lowest was in the supratentorial brain. ADC values declined the fastest in the cerebellum, followed by the thalamus, the slowest was in the supratentorial brain. The ADC value was the highest in the supratentorial brain, followed by the cerebellum, the lowest was in the thalamus. Conclusion: Regional differences in fetal ADC values and their evolution with gestational age likely reflect the differences in brain maturation. The maturation of cerebellum and thalamus is earlier than supratentorial brain.
