独眼抗青光眼术后并发性白内障粘连小瞳孔超乳手术的疗效与风险

摘　　要：目的探讨独眼抗青光眼术后并发性白内障粘连性小瞳孔手术的可行性。方法选择46例独眼抗青光眼术后并发性白内障粘连性小瞳孔患者进行白内障摘除及人工晶状体植入手术。观察术后视力，粘连瞳孔改善情况、眼压、人工晶状体位置、角膜水肿、晶状体后囊完整情况等。结果术后视力均有不同程度提高，裸眼视力：0．6～1．0者27只眼（58．7％），0．3～0．5者8只眼（17．4％），0．05～0．3有10只眼（21．7％），〈0．05者视力为指数／眼前1例，患者脱残率77％，脱盲率98％。术后眼压均恢复至正常。主要并发症有角膜水肿、后囊膜破裂、术后葡萄膜炎性反应及瞳孔变形等。结论对独眼抗青光眼术后白内障患者采取认真的术前评估，选择好适应证，手术时机，制定个性化手术方案，手术是安全有效的。Objective To explore the feasibility of the unique eye cataract surgery. Methods Forty-six cases of unique eye cataract accepted ECCE or phacoemulsification and PC-IOL implanta- tion. Results After operation, eyesight were improved to varying degrees, to take off the residual rate 77%, blind rate was 98%. The main complications were corneal edema, posterior capsular rup- ture, postoperative grapes membrane inflammatory reaction and irregular pupil etc. Conclusions For unique eye cataract patients, as long as we take seriously, safely, responsible attitude, correct preoper- ative evaluations, choose a good indication, the timing of the operation, formulation personalization operation program, and surgery is safe and effective.
